I wonder what the alternative, transparent Health Rosetta crowd would have to say about working with hospitals for clinical optimization. I love the idea. I think facing the problem head on is the best strategy. The best way to change healthcare in my opinion is by making everyone's voice heard while minimizing the money they could lose. I would like to understand the underlying benefit to the system of focusing consulting on that "issue" of variability; It's a big pain to the consumers, but does this really impact the providers and systems?
It allows the hospital system to succeed in a financial environment where they collect premium/take on financial risk… rather than just bill fee-for-service. Hospitals that have their own health insurance plans and collect premium have been much more financially stable. E.g. Kaiser, Intermountain, UPMC, etc.
You’re talking more about admin here not actual consulting. Biggest consultants are WTW, Mercer, Aon, Gallagher etc. nobody managing a large heathlb plan would call any of these for strategy

The unwarranted variability of care is the problem. Local expertise is variable. Most guidelines are class 2. About 10%of the guidelines are class 1.
Shared decision making accounts for some of the appropriate variation in care.
