I think part of it might be because ever since he was four years old he was called Deku, and the only person that actually called him by his real name was his mother. Then when he went to UA Bakugo called him, Deku in front of everyone, so they all assumed that was his name. Although he gave them permission to call him Deku, it was more out of habit of letting other people call him what they want then out of it actually being a name that he wants to go by after all these years of being called mainly Deku he now registers that is his real name.
