    06:20 the mistake I usually ended up doing while splitting the tooth ... I trace the buccal groove down apically ending up weakening my mesial root while splitting ... then I learnt tilting your handpiece slightly distally while splitting did the job

      Same I admire him a lot too he’s my favourite oral surgeon here on RUclips and also I love my oral Surgeon Dr. Kerr of Interface Oral Surgery Centre here in Canada 🇨🇦 as I’m trying to sleep tonight I’m still hurting a bit in my lower right socket it’s really sore and I still feel a little bit of numbness still from my wisdom teeth surgery back in April on Thursday, April 18th 2024 but I’ll just try to drink some water and try sleeping off the side where it’s sore 🤗🩷🦷🦷 @OnlineExodontia

    I don't fully understand the part where you mention cutting off the crown. @ around 5:00 min you say cut 2/3 way through then come from the buccal and snap the crown off.
    Can you explain that anyway by chance?

      Imagine doing a class V preparation, so lay your handpiece on its side and cut through the buccal surface. Go until you're almost through the lingual, but keep a shell of tooth there (same on the mesial/distal so you don't damage the adjacent teeth. Then insert an elevator into the slot, twist it to pop off the crown. Alternatively, you can sink a surgical bur down the occlusal and just section the tooth without removing the crown first, just there will be times where you inadvertently snap off the distal crown when trying to deliver the mesial root or when trying to separate the two halves of tooth. Hopefully that help! All the best!

    Ok so no matter what a dentist can get the roots out right? I'm getting more molars removed, slight curved root. Terrified they'll never get it out. Just anxious

    Which type of elevetor used to splitting the roots?!

      This is a 77R elevator. Any elevator that fits nicely in the section works just fine and frankly if you section far enough, lots of times when you elevate the mesial portion, the tooth will split and the root will deliver without first splitting the tooth from the center. All the best!

    I was working for crazy dentist he never let me use the hand piece to sectioning the teeth even if the roots are covered by bone ! He want me to use only the elevators and he instruct the dental assistants not to give me the surgical hand piece to make my job harder

      Definitely crazy. Haha! Perhaps they had their rationale for this, but I'd be surprised if you found an oral surgeon that never used their drill. Shortening the procedure has been shown in the literature to have a positive effect on healing / edema even when a handpiece is used to do so. In other words, it may seem more traumatic to cut bone to get a tooth out, but in reality, if you're doing it correctly, you're doing your patient a favour. All the best!

      Absolutely! I prefer skinnier burs in most situations (favourite is the 700) but if the 557 is what you have then go for it.

    Reflecting lingual side flap is quite not advisable ,while using air rotor for sectioning of tooth as air emphysema chances are quite higher.Buccal flap reflection is enough

      Thanks for the input Somnath. I almost always do these procedures 'flapless', but you raise a good point that flaps on the lingual are generally less desirable and not necessary for visibility. If you're referring to this video in particular with your comment I should clarify that you're not lifting a flap, but rather releasing the lingual collar of tissue. Also a good idea to avoid air-driven handpieces while sectioning and if you are using air-driven handpieces, ensure they are rear-exhaust. All the best and thanks for watching!

      If doing this, you must be sure to use a handpiece designed for surgery, not a standard high-speed handpiece. The best one I have found is the 45 degree Ti-max Z45 from NSK. All the best!
