  • Post-Tonsillectomy Bleeding
    How likely is it?
What can be done about it?
    How do we avoid it?
    Summary:
Virtually no blood loss in the operating room.
Around 3% chance of bleeding at any point during the 2 week healing process. If this happens, bleeding will usually stop with ice chips, ice water.
Around 1% chance of requiring cauterization or stitch. Sometimes will require trip back to the OR.
During the 2 weeks post op, do not fly on airplanes, go out on boats, or venture too far from health care.

I was part of the small, unfortunate percentage. I experienced arterial and venous bleeding at the same time. One was happening lower down my throat, which was awful. I went into surgery and stayed at the hospital for 2 nights while they monitored my recovery. All is well now! No more infections. Although my experience was VERY traumatizing, I would do it again. Thanks for the great video!

Dr I just want to say thank you for posting these videos. I watched them prior to my surgery and have used them for reference during the recovery period. It helps alleviate some of the anxiety of the process knowing things I am experiencing are normal. Tomorrow I am at day 7 from the operation. Pain got worse today as I think the scabs started to fall off.

Today is 12 days since my tonsillectomy. It was by far the worst and most painful experience of my life. I don't know if I would do it again if I could go back in time... Up until day 5 I did nothing but take pain killers and sleep. I tried to eat solid food because my Dr told me to. He said it would help remove the scabs and prevent bleeding. Early morning day 7 I took a pain pill to help with sleep and started choking. Ended up spitting out lots of blood, then the bleeding would calm down and then start again. This happened three times in about 45 minutes. Then I started throwing up blood and went to the ER. The whole ordeal was quite scary. I spent the day in hospital and now I'm almost 100% recovered, only a small amount of pain, no need for medication any more.

Dr. Liu, I wish our ENT had properly explained the risks more clearly. I feel like being in the 4% and then the 1% took us completely by surprise. On day 10, my then 6 year old daughter had an arterial bleed and was swallowing the blood as it was at the back of her throat and naturally had nowhere else to go. The paramedics couldn't see anything but we transported her to hospital via ambulance anyway. She was given zofran as her stomach was tender. Little did we know what was happening at that time. Two hours later she began vomiting copious amounts of blood. She was rushed to the ER. After transfusion she was fine but absolutely traumatized by the experience, as was I. I cannot encourage parents enough to keep close watch of their children. Yes, it's rare, but it happens. If bleeding occurs you need to access an ER immediately.
Thankfully she is fine now. 9 years old, no longer traumatized, and strep free for 3 years. Prior to that she was sick every month and it was so taxing on us all. The benefits of removing her tonsils and adenoids have been numerous.

I would like to know if you can help me with this one, my case is completly different. 3 days ago my girlfriend got a bleeding from her Tonsil that last around 3 to 4 minutes (she was spitting fresh blood) honestly it wasn't a lot but also it was not something normal... so the thing is that she doesn't have any surgery recently (she removed her tonsils 15 years ago!)... My girlfriend is 26 years old and she doesnt drink or smoke (she is a singer though) we had 4 weeks a go a Commo Cold but it last just 1 week and then everything was fine... this bleeding came from nowhere (right after she drank a hot tea, like she does every day) then after 2 hours she got again another bleeding but like 2 minutes and stoped (she also got a hot beverage in that moment right before the bleeding started, We live in China so it is very difficult to communicate with the doctors I will like to know if you can shine some light on it? sorry for my english it is not my first language, ... anyways we went to the hospital they didnt know how to explain it and they sent us home.. she hasnt bleed Anymore (this last 2 days) only drinking cold stuff, she got a blood test and everything came Clean (even a coagulation test).. the doctor just read the blood test he said it Looks like a "virus" because some things are slightly High but very very little.. anyways thank you if you can help I will appreciate.

There are many blood vessels in the nose/mouth/throat that can pop and bleed. This is more likely to happen when there is inflammation (like an illness). It is entirely possible that this is what happened and it never happens again.
And it's also possible that there is another explanation. An ENT might need to do a thorough evaluation and possible scope to make sure there isn't anything else going on.

  • @seanmccleary1

Unfortunately was in the 1% on day 12, which was surprising to emergency staff as this is considered rather late. Woke up in the middle of the night choking on the scab/ blood, lost a lot of blood and spend an hour or two in the hospital coughing up blood clots and blood which was awful and it wasn't for stopping. Ended up having to go and have the additional surgery, and spend the night in hospital for monitoring purposes, 4 days post op from this, recovering well and experiencing very little pain.
Interestingly, the side that bleed was the tonsil that I often had the most problems with pre-operation, causing it to be heavily scared and was the most painful post surgery.

My sis was that one percent, scariest moment to see my sis like that but I had to look brave for her, the worst part was, was the night she vomitted blood and woke me up for it, my parents thought to give her cold things like ice cold water, popsicles, things to make it stop...it did but the next night after I had slept with her in the living room to watch her it happened again, even worse that time so they took her to the emergency room but they wouldn't let me come considering that I hadn't slept since I was watching her the night before(staying up on my phone beside her) so I stayed home but yet again stayed up; to text them for a reminder if she was alright cause they had to take a bucket and a big cup of ice and water to slow down the bleeding while they were driving. I then waited for a reply to see if she was alright, which I later got but I think the most frightening part about those nights was when I would be standing by her while she was hunched over the toilet bleeding from her mouth like a slow water faucet while I was shivering but still keeping my composure. The doctor had told my parents that if they didn't take her to the hospital that night than she would've died to bleeding since it wouldn't have been semi controlable anymore. She was really pale for a couple days but I just thank God for watching her during the procedure that she went through the second time and to thank him for her being here.(It was an artery that was bleeding btw)

Probably because while sleeping you went too long without hydrating your throat. It's good to wake up every hour to drink water. When your throat gets dry it tears the scabs and causes bleeding. Some use a humidifier to keep the air moist, it helps too.

@@ranjanashukla3046 I know it's a tad bit late to reply but if you are still confused as to whether you want to have tonsillectomy or not then do read this.
First of all its important to know about your tonsil stones-
1)do they form on the surface of the tonsil and are easy to remove?
      Or
      2) do the stones form in a hard to reach cave like area formed between the tonsils and the surrounding tissue?
You can find this out by using the tip of your tongue to navigate towards the tonsils and to try to feel for tonsil stones or just understand the tonsil surface and its geometry/topology/shape and size.
If your case is of the first type then a slight change/alteration in habits related to oral hygiene can help, that is - 1) avoid rinsing your mouth with water upto atleast 20 minutes after brushing just spit the tooth paste out and that's it (only use drinking water to rinse your mouth not tap water, because tap water can contain excessive mineral compounds such as calcium and magnesium)
      2) you can apply toothpaste to the tip of your tongue and then use the tip of your tongue as a tooth brush to clean the tonsils and the surrounding area.
      3) tilting your head to the sides while gargling(to gargle the tonsil area) but be super careful.
If your case is of the second type and bad breath is really troubling you (which is normal) then tonsillectomy is the only permanent solution, but do find someone who is experienced in this... I assume you're from India- so the tonsillectomy operation would cost around 30k to 60k depending upon the hospital, location, method used etc.
Just make sure that you have a week off post surgery, let the surgeon know two weeks in advance that you're willing to go ahead with the operation and ask him/her about things that you should avoid leading upto to the day of the operation (there are certain medications that you need to stop taking weeks before being operated)..
      Hope you find this helpful
And p.s if you're worried about whether removing tonsils could hinder your throat's immune responses then don't worry as there have been studies done regarding this you can read online which suggest that there is no evidence of a significant clinical effect of a tonsillectomy on the immune system.

@@akashawasthi8129 thank you Akash 🙏🏻.. I truly appreciate your response and time taken out by you to write this!! … I fall in the 2nd category wherein my stones are hidden ( formed in the caves deep) and are visible only wen I try to press and scrap them out from the sides of the tonsils . It’s been more than 2 years now.,currently I’m taking dr batras homeop medication which is providing relief to some major extent , however I still hv to clean the tonsils every week which I hate .. hence I hv been planning to get tonsils out but I’m so petrified of the recovery process as an adult that I keep pushing it forward every year . I’m not certain about tonsillectomy ☹️

At day 8 I bleed all through the night coughing up blood but because I was so tired I didn’t realise until it had been heavily bleeding for hours. I ran into my mums room and we went straight to the hospital. From that hospital they sent us in an ambulance to a specialist hospital where they had an ENT specialist surgeon. I got put into a ward for a night and left the next day, however because of the severe loss of blood I felt extremely week for about five days after. Overall though it wasn’t too bad I think it was worth it for the amount of time I had my tonsils out.
The only extremely bad thing was the fact that while I was at the hospital in total I had at least 16 tubes of blood taken out of me to test for diseases and get my blood type in case I needed a blood transfusion

Day 7 and I woke up this morning because of the blood going down the back of my throat, I kept swallowing it which made me feel really nauseous while my mom was on the phone with my ENT. I threw up and the only thing in my stomach was water and A LOT of blood. Throwing up made it bleed more and when I started to gargle ice cold water it started to bleed even more because I wasn’t gargling it for long enough. Eventually it stopped bleeding but 1 hour later I threw up again and the blood started pouring again, quickly stopped it with ice water. Really hope it doesn’t bleed like this again or else we’ll probably have to go back to the ENT.

The second time was definitely more scary as it was pouring out of my mouth pretty fast. Both times I went to ED and they managed to stop the bleeding conservatively without surgery. I was in there for a few days each time. Im on day 14 now however not out of the woods yet. They sent me home with medication to clot my blood to prevent bleeding. I was told it was because of an infection and there was nothing i could of done about it.

    • @GeneLiuMD
      @GeneLiuMD  5 лет назад

There are many reasons for this, nearly all of them very temporary. It is really common to have alterations in taste for a few days or weeks. Multiple procedures makes it more likely. Your surgeon will have more insight into your specific situation about what to expect and why it happened.

The doctor said that will cause stuffiness afterwards. Are my worried unfounded or valid?

    • @GeneLiuMD

I just had a tonsillectomy 5 days ago. I’ve had a lot of pain but it’s manageable.
I’m concerned because I clear my throat ALOT due to acid reflex, and even though I’ve tried my best to not, I’ve done it a few times during post op recovery. Also, one of my tonsil beds has a bright red streak that looks like a cut. I haven’t spit out any fresh blood tho, only old brownish blood. But should I be worried about the red streak? I’m just not sure if it’s normal or not.

    • @GeneLiuMD
      @GeneLiuMD  5 лет назад +1

It's hard to know from your description. Certainly, fresh blood would be concerning. Other times, a slow drip could collect blood in the stomach and cause nausea. If you vomit up brown/black "coffee grounds" appearing material, it likely means there is ongoing bleeding. Also, blood further down your intestines can cause cramps, loose stools, diarrhea that can be black or "tarry".

@@VALPARAISPECIAL Hello! I’m not really a doctor but what helped me was placing a frozen bag of peas (or anything cold really!) over my neck and throat, this then constricts the blood vessels and stops bleeding. It’s scary lol so I feel for you. I think after it happened I was really wary about what I ate for a day or two as I didn’t want to knock the clot out of place and have it happen again. Try cold yogurt, ice cream and mousse as this is soft and cold, definitely keep away from anything warm near your neck and throat and if in doubt contact your surgeon :) It also helped me to not talk or cough/anything that would make it move. Hopefully there’s light at the end of the tunnel for you!

There are many reasons why someone develops recurrent infections. (ex: underlying immune deficiency, poorly controlled diabetes, frequent exposure to pathogens, rheamatologic disorders, etc) Generally, it would be useful to see a primary care doctor to evaluate for these other medical conditions. If nothing is found, then a meeting with an ENT may be beneficial.

But later in at nighy I had to sneezed and that's when I started bleeding. It bled for 10-15 mins then stop after I drink cold water. Unfortunately I have no popsicles or ice cream. It tends to bleed every 1 1/2hr. So for 4x. Is this normal?

I strongly recommend calling your surgeon's office ASAP. If you aren't sure, going to the ER is always the safest route. There's no way to predict what may happen next. We usually want no bleeding at all.
Bleeding is a risk that can happen at any point from the OR to the time where the last little bit of scab is gone (typically 2 weeks post op). We don't like any bleeding. Sometimes a little bleed is all that there is. Other times, little bleeds happen before a big bleed.

@@GeneLiuMD ... Thank you Dr. Liu... After asking you the question I ended up going to the ER. I was bleeding really bad. Come to find out my left tonsil wound opened. I had to rinse my mouth with ice cold water to stop the bleeding. No antibiotics given. Since the bleeding stopped they recommended rest, keep hydrated, ice chip, sit upright position, etc. They said I have to let the body heal on it's own. I guess my healing is backwards. Heavy bleeding now n light bleeding later.

@@lindalinda5568 Glad you went to the ER. Antibiotics don't do anything for bleeding either way. Definitely take it easy, ice cold, hydration, upright, etc... are all the right things. There are times when someone might need cauterization or sutures if it keeps happening. Fingers crossed it doesn't bleed anymore. Best of luck!

I was in the 1%. We didn’t know I was still bleeding. What happened was I started to smell. Blood was building up in my stomach. My mom called the dr he told her to bring me to the hospital, guess because I was moving around by the time we got to the hospital blood was coming out of my mouth. They turned me on my side and rushed me to emergency surgery. The dr said there was so much blood in my stomach. I was 11 it was horrible.

Does anyone know anything about taking steroids post op? I’m on day 4 and my doctor put me on course of steroids on day 2 because my throat was so swollen that I was hard to even take sips of water. I didn’t want to stop drinking water cause I know how it’s important. And I chocked a few times trying to take the pills. So I called him and he prescribed me methylprednisolone to bring down the edema and to help with the pain, but now I’m freaking out because I heard that steroids can delay healing and may cause bleeding. Anyone please tell me what you know about it?? I mean it’s hard to think my doctor would prescribe it to me if there were risks of bleeding but that’s what Google told me and I’m freaking out. And I’m only starting day 5 this morning ! 😥

Sorry to hear about the bleed. It's always a scary thing to be going through. The overwhelming likelihood is that it won't bleed, but there is always a small chance. Take it easy and good luck.

Bummer. Did you talk to your surgeon about it? Just make sure that you keep him/her in the loop. And if the bleeding is at all significant, you may need to go to the emergency room. You can ask your surgeon for how and when to notify them and what to do if it happens again. Good luck!
