August 22nd of this year, my mother passed away as a result of multiple traumatic injuries as a result of a head on collision. I worked with her, her boyfriend insisted he used her car that day to go home (we lived about an hour from work, he lives about 10-15 minutes away) so she rode with me that day. If he never bugged her for the car I would’ve left earlier and the traffic wouldn’t have been as bad and if I did get into the same accident, I would’ve survived at least. Anyway, I was rushed to the hospital with a collapsed lung. Blisters on my lungs, one popped from the airbags leaving a tear in my left lung. When I got to the hospital they immediately cut my only remaining piece of clothing off (my underwear) and they immediately told me my lung was collapsed and I needed a chest tube inserted which involved essentially going under the knife. This right here was super accurate and I even looked away the whole time and cringed at the sound, just brings back memories. 5 nurses had to hold me down while 2 worked on getting the tube in. I’m 20 and they didn’t wanna break bones so they used an oxygen hose, spliced with the chest tube, with a connector and pinched off with sterile zip ties so it didn’t come apart. I was on pump for 7 days before they realized I needed pleurodesis and a proper thoracostomy. I was discharged after 9 nights and 10 days. They glued everything shut but stapled the incision shut where they placed the second chest tube so I have 3 decently large scars.
With very well placed local anesthesia (skin injections) you can eliminate a lot of the pain but it’s still an unpleasant experience. If possible I like to give ketamine or at least an opiate And something with an amnestic effect.
When I had my ankle popped into place, they gave me something where I wouldn't remember either. But you're awake for it just can't remember. Craziest thing.
@@matts1451 Yes, ketamine is a great choice for a thoracostomy from a traumatic source (keeping in mind that you should be prepared to deal with a possibly more likely emergence reaction given the likely state of mind of the patient during the injury). It's a very painful procedure (as I was hamming up in this simulation). There are the rare cases when because of hemodynamic instability you will have your sedative/analgesic/dissassociative choices limited but you should be able to work around almost all of them.

@@matts1451 I have not heard of it. It would seem to be limited to transcutaneous placement from a time constraint perspective and that might interfere with the logistics of the procedure. That being said, if somebody has figured out how to do it safely and effectively, I'd love to hear about it.
I feel your pain I have had four tubes placed one placed where you had one and one in my sternum there isn't any amount of local anesthesia prepare you for that I don't care what they say. The other two I had right below my shoulder blades

ooof.. that looked pretty brutal ..the way to push the tube in place.. you're sure you didn't cause tissue damage to the lung? (I know it is a simulation.. but it looked rough)

Yes, In reality, I prefer to use the Kelly clamp to smoothly direct the tube cranially just along the inside of the chest wall. Good point!
