I've seen a helm for shinobi called the sagasu helm, but I haven't found any information about it. How do I get it?

No not really, I mean you could look at perks but nobody really cares about perks especially since endgame in For Honor is fashion. So essentially no it's entirely up to your skill to determine whether you win or lose a fight
