You are an amazing runner. I'm a 49 year old runner that suffers with piriformis/glute/hip pain. Like yourself it flairs up from time to time. I see a physio and follow a prescribed strength routine in the gym twice a week but it still gives me grief. Is there any particular exercise or something else you do when it flairs up that you would recommend trying?
Thank you. I've had some real issues with piriformis syndrome in the past to the point where the only option was to stop and rest. I've managed to keep on top of it relatively though recently the same as you with sports massages and S&C work. One exercise in particular that has helped is hip drops on a step. Stand sideways on a step and let your one leg drop down using your full hip/pelvic range without bending your knee and then lift back up. I try to do 20 of these on each side every night before bed. Another is some hamstring extensions lying on your back followed by some nerve flossing. A good regular stretching routine with some yoga added in can help. I also use a foam roller before every run to loosen everything up and then use a massage gun to get into the spots that are particularly sore or irritating after. Hope that helps and you manage to keep it under control. Unfortunately age catches up with all of us no matter how far we try to run away haha

Thank you Thomas. Those glute issues plagued me for most of 2024. Fingers crossed I can keep them at bay a bit more this year.
