Autocomplete GoHighLevel Form Address Fields with Google Places API

I just paid for the GHL Saaskits and i used the code from the snippets library and its not working for me as soon as i type the address the the form turns grey and freezes can you help me please

Code snippet does not work. It looks like autocomplete is working on the form, but when you check your contact in High Level, the city, state and postal code are all missing.
