    I was diagnosed almost 12 years ago, with a Gleason of 3+3, so I am following Active Surveillance. If and when the time ever comes that I need treatment, these videos have been extremely helpful in letting me know what to expect with the various treatment options. As I watch these videos, Alex has a knack for asking the follow-up questions that I think of. Thank you for this valuable series and I know I will be armed with a lot of knowledge when speaking to a urologist.

    Thank you for this great video. I am scheduled for prostate removal on March 6th. My only other option was hormone therapy with radiation where I am not too excited about hormone therapy. I am 71 years old and I also have Peyronie’s disease which already causes some problems. I will be asking my doctor about all this. Thank you so much for this video.

    I just wanted to thank you. I am 52 years old and I had my prostate remove in October 2022. In my opinion surgeon do not provide enough information about what going to happen to our penis. Unfortunately patient are not able to prepare for what about to happen to there sex life. I had intercourse until the day before surgery, then it was like they turn a switch off. Incredibly difficult for sexually active man to experience this from one day to the next .😢 I was fortunate enough to come across one of your previous videos on the importance of penile injections. 🙏🏻Your video made me realize that if I was not going to be aggressive in treating my ED after surgery that not nobody would. With that said I went back to my doctor 2 months after surgery and told him that wanted penile injections for rehabilitation. It been 1 month since I started the injection(trimix), and it quite challenging at the beginning, but it also very nice to see my penis erected. By the way it’s not a erected penis that make you a man, it facing your challenges that make you a real man. Thank again and God bless.

      I'm booked in for March. I don't think there's any way to comprehend it. I feel like I'm signed up for entry into a Nursing home really. The time-scales are un-imaginable. ~= 3+ months for fixing incontinence and ~=1+ years for erections, and that's considered a success, and you're one of the lucky ones. Unfortunately I do agree with the surgeon, if I leave it in there I've potentially got years of whack-a-mole to keep on top of it.

      How many units of trimix did u have to inject??
      Im 64 had hormone therapy and 28 radiations i tryed 30 units and it didnt work i will try 40 next.

    Thank you for addressing this issue. I think that much of this lack of discussion prior to and after surgery is that men are not comfortable in discussing their sexual issues with other men. It may also be a difficult topic because, my understanding is, that nearly 70% of urologists are female. Additionally, it seems to me that prostate cancer needs to be better addressed by mass media. The percentage of men diagnosed with PC is nearly equal to the number of women diagnosed with breast cancer. Breast cancer is addressed in detail in mass media. LastIy, I feel that all patients need access to support groups for men diagnosed with PC both before and after surgery.

    Radical Prostatectomy in 1996. Pills were not out yet. Urologist never informed, nor referred me to another doctor for ED. Told to wait at least two years. Nothing. Never heard about “Penile Rehab” until a couple of years ago. Too late now, after 26 years of no erections. I would take my cancer-addled Prostate back in a New York minute!

    After surgery the doc was pretty much MIA, gone. So much more to this other than cut and run as I am learning. Very enlightening. To complicate this I experienced scrotal lymphedema, awful stuff, totally unexpected as I didn't know that was even a possibility. Getting better on its own to a degree, but with no help with that either. Feel abondoned by a doc that may know surgery but has little concerns for the aftermath.

      @@georgewiel Saratoga NY. I think the surgeon did well, just no time to learn about all of the rest of the aftermath that goes with it.

    Thank you for creating such informative videos. I am just starting on this journey with a diagnosis just 11 days ago. I feel very fortunate to live in a country (Canada) where treatment is available. These videos are helping build my questions that I have for my Urologist so that I make the right decision for me as to a treatment plan.

    I'm 55-year-old Veteran, just diagnosed. Three spots of 3+4 7 one of 3+3 6 and one spot 4+3 7 Intermediate all on one side of the prostate, other side biopsy's clear. My Urologist is a very nice, good doctor, and told me "Men my age get the surgery!". Briefly mentioned radiation for about 1 second as an option. Did not speak much of the side effects and when I asked about erections, incontinence, told me only 10% of men after surgery have issues? After watching many PCRI videos I am going to peruse radiation for my treatment. I haven't seen my Oncologist yet, but I am praying they can help me with radiation treatments. I want to get rid of the cancer but fully enjoy my active life after, no matter how many years I have left. For you men that have had the surgery, I applauded your bravery and hope you have a full recovery and great cancer free life! Thanks Dr. Scholz and PCRI for your RUclips channel.

      Hi i m Chris , diagnosed like you 3+4 3 spots 3+3 one spot as one 4+3. Age 57. Heard that side effect from RT go up to 20 years after treatment

      @@egman5225 i didnt do any treatment yet but surgeon says that with my age and physical state and sexual state i wouldnt have too much problem recovering. Small tumor. Small prostate psa is going down so it s a slow growing. He will do the ‘nerve sparing’. Havent seen a radio oncologist yet

    I was 68 when diagnosed with an aggressive PC. I had a PSA of 38, 11 cores out of 13 were Gleeson 3+4, one core was Gleeson 4+5. I also had distant mets to lymph nodes in Aorta and clavicle region. Both Oncologist and urologist agreed that the best course of action was a radical prostatectomy with seminal vessels removed. Following RARP the urologist put me onto cialis 5mg for about 2 months after which he stated that their was no further benefit to me as cited by recent research. However another referral to a penile urologist (who specialises in implants) placed me onto cialis 20mg 2-3 times per week for at least 12 months. At the same clinic a nurse with extensive experience with prostrate cancer said that cialis 5mg daily was best for my penile "health" and not the 20mg variety! Penile injections were discussed but this was to be delayed until after my chemo treatment had completed. My original urologist who did the surgery was hopeful that I would recover erections after 12 months whereas the "implant" urologist stated no chance of this happening and that implants were the gold standard. I am as confused as a blind dog in a meat house!!!!

      Sounds like the implant guy wants to do a procedure. The implants can’t engorge the head of your penis or the shaft fully for that matter. Give yourself every opportunity to get an erection on your own. BTW I’m an Anesthesiologist and I’ve seen lots of implants.

    Good morning I just wanted to tell you how much this page has been instrumental in my prostate cancer journey. I had subscribed to this channel and rang the notification bell because every video that I've watched in relation to my particular situation has been instrumental educational and so helpful. I'm so glad that this is available for us men. I do want to share some good news with you all so my Gleason score was a 7 and jumped to an 11 in 1 yr. Plus 1 I did the biopsy 11 out of the 12 samples were contaminated with cancer. I had the prostate cancer surgery with the divinci robots method 2 weeks ago. My recovery has been great, Again thanks to your page and your videos I've been following they just helped me to before and after the surgery. And I think that this video is very important to me because I've been wondering what's the next step or phase and I...I just want to say thank you for great information. I also want to give you a great report that they were able to remove the entire with nothing left behind and the lymphnods that they're extracted showed that there's no cancer (benign) so I am cancer-free! I am so grateful to God, the skills of my surgeon and the love and support of my wife and all family and friends in both sides. Again I am grateful for your videos and information. Thank you for educating us.

    My surgeon, or the E.D. specialist he subsequently sent me to, have never shared this information. Once I told the E.D. guy I wasn't interested in an implant he said, call me if you need me... I'm 2.5 years post op.

    The only problem is that if you are on ADT plus possibly abi-pred post surgery, you will not only have no libido, but little prospect of a "spontaneous" erection as long as you are on those drugs, maybe as long as 24 months post surgery if you were "high-risk" Gleason Grade, plus a testosterone recovery period. That's a lot of Trimix shots at 2 per week, with little, if any, emotional libido desire to have sex, and of course no ejaculation on climax possible. After a few Trimix shots, I said "Who am I kidding" to myself and just focussed on staying alive.

      Totally agree and it happened to me.
      The injection was easier than it looks. Worked…but a little too we’ll and had a painful erection almost 3 hours. I became shell shocked and with no libido, was not interested in continuing. And the cost for the injections was astronomical.

    It helps if I know what questions to ask .
    But I see your point . Great job folks .
    Um , for the rest of us , personally, I feel lousy if I go for a long time with out an errection . My attitude takes a dive too , depression sets in and that triggers girlfriends . Because believe it or not , when sees that monster stick its head up , it makes her feel good that she caused that to happen . Yah really !
    Our self esteem helps her self esteem . But like every other guy , I’m shaky about needles in my penis . But I’m going to talk to my urologist anyway .

    I asked my surgeon for a referral to a urologist that specializes in men’s sexual health. He routinely works with men who had prostate surgery. He recommended injections. Injections are not a big deal and the results have been worth it so far. My urologist believes that natural erections will return over time. Be brave! Don’t settle for a sad life!

    I have been through a prostatectomy in the last 72 hours
    I think the tremendous and brutal insult to this area (years ago, it was much worse) during and after surgery is the cause of incontinence and erectile disfunction. Rigid catheter, so many days with abdominal pain and deposition difficulties causes damage in the area difficult to recover.
    Cure will come only when we know the cause and try to avoid it

      How are you doing now? I’m 10 months post surgery and doing well. I did 8 weeks of pelvic floor therapy which took care of incontinance 100%. I can now go 4-5 hours during the day without having to urinate and can sleep through the night sometimes. As for ED, it had been soul crushing. My wife and I had an active sex life b4 surgery and doing without has created a challenge to intimacy. I started injections two months ago and all I can say is that the shots aren’t a big deal and the results are worth it. The urologist believes that this will bridge the gap until natural erections return over the next 12 months. Good luck!

    I am using the injection therapy and it helps a lot! I use viagra on days that I do not inject. I wish we could cure incontinence which is my real big problem.

      Update. Kegels did not seem to help much. Imipramine reduced the incontinence by a third to a half. 2 years after prostate removal, I still had bad incontinence. Then I got a male sling operation and this cured the incontinence.

      I had radiation plus ADT but the ADT made me practically impotent. I’m off now and starting to have erections but worry about long term effects of having very few erections for 3 months.

      Follow up: met with my Urologist yesterday. He said shouldn’t be any effects. Says I’d have to go six months to a year without an erection before I develop atrophy

    Be careful with Trimix. I ended up with priapism from trimix. I had to have an aspiration irrigation after having trimix. Not sure how long ill be able to have on demand erections. I take nitric oxide, L-Arginine. I have the best erections I’ve ever had in the morning, and spontaneous erections. But not on demand erections. Does having an aspiration irrigation of the penis damage the dorsal nerve ? Does that take time to recover so i can get on demand erections ?

    I was not immediately offered penile injections. Is 3 months to late to start after nerve sparing radical robotic prostatectomy? Cialis is doing nothing.

    This is the first I've heard of this as a therapy, I am just at two years post op and I can get semi erection, would this therapy be a good discussion with my urologist?

    Wow. I would think of all disciplines, Urologists should be prepared to help a man recover his ability for an erection using injection therapy. Not sure an Internist would be ready for that discussion, especially with limited visit times due to insurance limitations.

    How long after having an aspiration irrigation of the penis does it take to get on demand erections again? I use a penis pump regularly. I also take L-Arginine and Nitric oxide. I do also take 5my of Cialis. My erections are the best ever, but they happen in the morning and spontaneously. Do the penis nerves get damaged during the aspiration irrigation of the penis? How long does it take to get back on demand erections ? Please, some real medical answers if anyone knows

    I am an 82-year old in-pretty good heath.
    I’ve read a lot about CyberKnive, TrueBeam & SBRT.
    My 12/2/22 Prostate MRI W/WO Contrast Impression
    1. PI-RADS 5 lesion in the anteromedial right transition zone with extracapsular extension.
    2. PI-RADS 5 lesion in the posterolateral left transition zone.
    3. PI-RADS 4 lesion in the posterolateral right peripheral zone.
    4. PI-RADS 4 lesion in the posteromedial left peripheral zone.
    5. Prostate volume is 68.89 cc.
    6. Morphologically abnormal left external iliac lymph node.
    My 1/9/23 PET/CT Impression
    1. Mildly enlarged prostate with multiple foci of intense uptake concerning for malignancy.
    2. Avid right external iliac chain lymph node. Metastatic disease cannot be excluded.
    3. Redemonstration of conspicuous left external iliac chain lymph node, which demonstrate mild uptake and are nonspecific. Attention on follow-up.
    4. Nonspecific cervical and right supraclavicular lymph nodes are nonenlarged by CT size criteria but demonstrate radiotracer avidity. Similarly, nonenlarged radiotracer avid left hilar lymph node is nonspecific and could be reactive in nature

    I wish my surgeon had not given me options after removal & demanded that I choose injection therapy for desired results and not given me options that did not work. I least I am still alive and married.

    I had my Prostate removed over 2 years ago and have not had an erection since I had it removed. What can I do to regain this function and maybe enlarge a bit . I heard that it takes 2 inches off your size to tie in the tube and veins .

    Good evening I had robotic prostatectomy 4 years ago. They had to do non nerve sparing surgery because of the amount of cancer I had. I was not informed of these penile injections. I have had ED ever since. Is there any hope with getting my erections back without another surgery?
