    I am a retired anesthesiologist and have had four lumbar fusions over twelve years for DDD and spinal stenosis/ fascet disease. Your videos deserve an academy award for factual down to earth understandable explanations for various spine conditions. Please keep up the great work you do.

    Thank you for highlighting the difference between peer reviewed research and industry funded papers. I have severe DDD and I read a lot of studies on the topic. It is often very difficult to even know who is behind the study or what their incentives are. This is SO important to understand.

    I’m 44 with DDD and more. Nothing has helped me and I’m terrified to have surgery. Walking and sitting is an issue. I can’t work out. I had to leave my job recently. I am so happy I found your page today.

      Omg, me too, I just turned 45, felt in great shape until a year or 2 ago, now my neck is already in Stage 3 and just found out I have degeneration in my L5, S1 too…. I don’t know what exercises to do or not do now 😢

    When I started having problems, surgeons told me to hang on because advances in artificial disc replacement were going to make them the next big thing . It's 20 years later and nobody seems to even mention that option now.

      A neurosurgeon where I live went nuts doing that surgery. He was sued by over 40 patients the last I knew. I thought it would be my healing too after decades of this. I am thankful I could not go do it. This physiotherherapy or with the blood platelets is the best thing I've heard.

    I was diagnosed with DDD. I’ve been a steel worker since I was 17yrs old. Pretty sure moving tons of steel for nearly 30yrs has caused by back problems.
    I met with a chiropractor, had no relief. I then went to a pain consultant doctor that did a series of injections. She did four rounds of injections (4 injection points each time) at my L5-S1 within 3 months. She referred me to a neurosurgeon. He had me get an MRI, immediately after seeing it said I need to stop working and get prepared for surgery.
    I apparently had severe Retrolisthesis, I couldn’t feel my feet. Walking became a challenge.
    It’s been 3 months since my ALIF, I was feeling good for the first 5weeks then the pain began to set in. I’ve started PT this week, it has had immediate results.

    I am so glad that I found your channel. You are very clear in explaining these issues to those of us that are experiencing some of these issues, but are not in the medical field and don't really understand. It is frustrating when you have pain and you have your doctor orders an MRI. When you go back to your doctor they don't explain the report clearly and slowly so that you understand what the MRI shows. Thank you so much for helping me understand my MRI report. Please continue educating all of us. You have a special gift of helping people to understand and make good choices in their care.

    I have been diagnosed with mild scoliosis located just above severe ddd in L5 s1, my family doctor says because my scoliosis is mild so he doesn't understand why there is pain in that area, only where the ddd is where the pain should be. Keep in mind the pain where the scoliosis is has only been around the last almost 2 years the ddd over 29 yrs but worse last year. Dr Su I am so grateful for your insight because my doctor will not send me to a specialist. The mri's 10 years apart show the severe deterioration differences use to be bulging now not much there at all between the L5 and S1 left. I am doing everything to alleviate the pain . One I am no sugar no grain way of eating. Doing core exercises, walking a lot, puppy pose and now he finally is sending me to physio.

    My lumbar and cervical spine are a mess. I have severe stenosis and 3 bulging discs in my lumbar spine. My neck is straight with disc disease. I’ve had MRIs and was immediately suggested to have a laminectomy and fusion for the lumbar spine. I refused both. I’m on 25 mg of Pregabalin at bedtime, I use a cervical pillow, a passive neck stretcher and a SI belt. All help but my back is getting more painful. I didn’t used to have back pain, but sciatica and hip pain. I see our cervical neck chiropractor weekly and ice when needed. Degeneration is so much fun! Thanks for the info.

      Any relief now am having sameness with burning sensations symptoms on both arm and feets and I have stopped taking pregabalin because it temporary pain relief and cannot leave my life on pain killers. Please share your symptoms

    How do you filter out B.S in a study? Patients can lie so is there some control procedure to consider that? The only thing that has helped me is routine light exercise. I had to be damn near desperate to give it a real chance though because of the pain i was already enduring. Surgery made it worse and drugs made me more sensitive. I had to be tough on myself. Thanks for an honest discourse of this issue, it was better than a 10 minute appointment.

    Loves ur videos alway watch them, why is it when you see a surgeon for DDS no one never talk about checking for Pelvic positions disorder like ApT which in a lot of cases is the main source of the pain and contributes to DDS , even physical therapy missed too

    I have 6 years of unsuccessful non operative treatment , I’m praying to get a surgery one day. I feel like I tried everything from steroid injections to a neurotomy and nothing works except the oral Prednisone I take for a couple of days before getting an infiltration which my GP refuses to prescribe. I’d take that 15% difference

      I have DDD, 3 drs all said it is inoperable. I had lumbar and cervical injections and had severely bad reactions to both. The only thing that helps me is pain meds which, good luck, finding a Dr that will prescribe. really tired of living in constant pain. Btw for some ppl like myself, steroids have nasty side effects worse than any pain med.

    Thank you Dr Su for making these videos. I’m an anesthesiologist and interventional pain fellow. I had reservations on provocative discograms especially tampering with a control disc which may be perfectly normal. Some institutions rely on them a lot. I felt that they were over utilized and possibly harmful. It’s great to know you stick to evidence based medicine. Keep up the great work. Cheers.

      I would not use a provocative discogram they are painful and they have been shown to increase degeneration based on the Stanford studies. However there is terrific evidence for functional anesthetic discography. It is low volume does not pressurize the disc and correlates with surgical outcome

    Really helpful thank you! Love your detailed description and openness about treatments. My spine doctor had zero to say about DDD. Basically the “there’s nothing you can do” and invalidated my pain by saying DDD doesn’t cause back pain. Beyond disappointing and traumatizing!

      I was treated dismissively by two spine surgeons at UCLA. Then two spine surgeons at Cedars Sinai, plus one other outside who I saw for a second opinion, said I definitely need the surgery.

    In 1994 at 38 years old my foot dropped with no relief from a decompression which was spine surgery #2. The foot has remained dropped. in 2014 I took a bad fall and subsequently was fused. Now My "good" side has a lot of, numbness R/T stenosis and bulging discs etc. I am a good weigh don't smoke and despite the dropped foot ride my bike, a lot. I have an inversion table , do hand stands and try to stay hydrated and active. My whole adult life, it seems, has been devoted to issues with my spine . A major drag.       My ex used flint rehab device and it helped with foot drop. Try to look into, usually used for those with stroke but many now covered by insurance along with physical therapy

    I have stenosis and degeneration l5 s1 causing severe sciatica. I've had 5 flarups over the last 4 years or so and gets worse and lasts longer each time it flares up. A month ago I got my first injection and unfortunately the pain became so much worse after the steroids. Therapy does ease the pain quite a bit but it's just temporary relief, every morning I wake up and pain is back to intense and I struggle getting ready for work, showering,dressing, tying my boots,ect. Very informative video

      Omg we have the same issue and I was reading that injections with steroids make the problem worse and now I am terrified to start this treatment with pain management, I don’t want to take more pain killers bc of the pain caused by injections. What should I do???? I can’t even do my job anymore

    In my case, had DDD for over 10 years and more. Recently tried a few de-compression exercises and they seem to work really well, especially for the lower back, just letting your core hang loose supporting yourself between two chairs e.g. Upper back seems more tricky but pulling back and down seems to work work somewhat.

    Great information. I found out a couple weeks ago from my chiropractor that I have degenerative disc disease In between the 6&7 vertebrae in my neck. Had my MRI this past Wednesday and it showed severe bone growth with a lot of calcification. He is sending my results to our neurosurgeon for a 2nd opinion to see if surgery is needed at this time or not. Until then, we will continue with adjustments, traction and massage therapy with the chiropractor and I will be going to another doctor for pain management using injections. It’s been quite a wild ride. Thank you for your video.

    I am currently trying to manage back pain as I wait for an MRI. I'm still very young, barely 30. Its a combination of so many issues with DDD showing up on an preliminary X-ray. Its a life long process and quite frankly exhausting.

    Disk between L4 and L5 is bad. The shot they give me gives me like 95% of my range of motion back pain free. Lasts about 8 months.

    My hips keeps falling out of place. It’s very painful. I am only 41. I used to be fit and very active and now if I sit down my hips is out. Doing normal chores is a nightmare! I went to chiropractor, physical therapy and my back is still crooked.
    I am trying one more thing and I will flip on every body because I can’t have surgery when I’m 70

    I have a question. I visited a chiropractor for consult after a diagnosis of lower lumbar spondylosis from L3 through S1 with degenerative disc disease ( age 68) and was given info on the DRX-9000 spinal decompression unit . What are your thoughts about this device ?

      There's minimal harm in using it with the problem being once you stand up gravity takes effect again.

    I have had disc herniation and high pain for the past year plus degenerative disc disease and a bunch of other stuff like scoliosis and stenosis. I tried anti-inflammatories and I got ended up with a gastric and esophageal ulcer and the pain was still there. I just tried cortisone injections in my spine and it didn't even touch the pain I have had trouble with cortisone injections in my knees and feet as well so this does not work on me. There is nothing left because the PA at the office said that there's nothing wrong with my spine and I should just go home. mri is full of problems but he thinks everything's peachy. So I can't even get up with the surgeon because PA blocked communication. Now I'm looking for another surgeon but half the surgeons don't even do some minimally invasive spine surgeries. Hi have to go from one spine doctor to another trying to figure out if they can do a surgery that can help me. Because not all of them do every type of surgery. So I stay home on pain medicines. I have no life and I'm in pain everyday. It's nice to talk about ideal situations regarding surgery and the choices seem to be easy. But in real life it's hard to get people to cooperate and be helpful. I really given up on doctors

    Can you answer this ? foe me.i had surgery awhile back and they tried giving me an epidural and it didn't take well needless to say they didn't do a patch job on me and for like 12 hours plus I was losing my spinal fluids can that have done more harm to me ? They had to come in the next morning and put 20 CC of blood to stop it so could that have done more damage to my DDD problem thanks waiting patiently Teresa.

    What I find so interesting is my Mother had severe DDD and zero back pain I have mild DDD on 2 discs and have issues sitting /working out - she at 74 had more mobility than me!!! so many factors affect back pain ...

    I just had an intervention done on my L4 L5 and L5 S1 that included ozone+blood plasma+collagen...its very early to say if it works

      Those treatments do not have any scientific evidence but may work and if it does for you that's great. As a disclaimer this is not considered medical advice and you should always seek advice of your spine surgeon

      @@andyl9900 Better, although I´ve always kept excercising as well so it´s hard to say what actually improved my condition and also, I went to chiropractor for the first time ever and it turned out my pelvys was twisted a tiny bit. This needs to be corrected first. You need to make sure your "base" or core is in line (both legs are the exact lenght) before you start any king of treatment. It is caused by muscle imbalance. Also, after let´s say 20-30 minutes of sitting, I get up and do some stretches and pelvys movements, or even traction if need to be. Also, long walks seem to be helping. You need to be moving as much as you can. Hope this helps.

      @@boriskrizan7447 Glad to hear you found what worked for you. May I ask what was the problem with your lumbar area that required intervention? Yeah I think muscle imbalance overtime might have caused one of my disc to degenerate. And I'll probably try those treatments you mentioned above before going into surgery, as pain level is stable at a 5-6 for now.

    Question for you Dr. Su. You mentioned in your video, "if it was your back you would try PRP". My question is, If it was your back, would you also use a combination of Mesenchymal stem cells injection with the PRP at the same procedure or would you just use PRP only?

    After 8 months of physical therapy I have been sent to pain management for my DDD from L1 through S1, currently I have 5 bulging discs and L4-L5 is unsurprisingly in the worst condition. I am getting the epidural steroid tomorrow and I hope for some relief. My personal case of DDD developed when I was 19 and I am now 20 so I am just a little salty about it being so early onset. For the past several months I have been entirely incapable of running or complex movement with rather annoying constant pain. I'm just wondering if surgery is the right option if I am unable to experience relief after my series of injections.

      It depends on what the imaging looks like. Unfortunately if you have multi-level to generation it is unlikely that you would be a surgical candidate. It's hard to know without evaluating the imaging. As a disclaimer please consult your surgeon

      At your age I would try everything short of surgery. They are developing new technologies including disc replacement.

    And what is done to determine if it is ruptured? I was told for 15+ years that my disc was "just" herniated here are some pills.... The pain got worse, much much worse over time. It was the discogram that determined that it was actually ruptured and not just herniated. Surgery was done to fuse and the pain was nearly taken away completely. How would numbing determine if ruptured?

      @@DrBrianSuTheSpineGuy I had numerous MRI's and they never determined a rupture, only said it was herniated. It was not until the Discogram and a CT that they were able to see that it was ruptured.

    i fell at work i have 5 discs that have moved and i have a collapsed discs hip pain knee pain c6 c7 i have a disc pressing into my spinal column all of the pt in the world will not change or fix this!!! in addition i have 2 pinched nerves one on each side of my lower back i have ddd on 5 discs my tailbone that hurt radio frequency simply caused a 5 day flare so im currently getting ready to have a sacroiliac nerve block injection

    I just had X-rays done a couple days ago and DDD was listed in the report. Also, L2-3 was listed as having vacuum disc phenomena and endplate sclerosis, plus I have a leftward curve in my lumbar spine. I’ve had pain, numbness, and tingling all down my left leg into my foot and sometimes on the right, and a persistent numb patch of skin on my left front thigh. PT has made zero improvement so far. I have a consult set with a spine surgeon in a few days and I’m paranoid that surgery will be a topic of discussion. 😱

    Can a tear in the rotator cuff influence negatively to a pinch nerve in neck? And can pinched nerves in neck make you dizzy?

      No it cannot but they can often overlap in terms of pain distribution. You should see a shoulder doctor and a spine doctor separately

    I saw a consultant today who recommended steroids injected into the L4 and facets joints in the area because I have running sensations in both legs and feet for 3 months now. There hasn’t been any pre injection therapy recommended. I hope this works for me.

    Physical therapy really caused more pain (3 times), the more I stand-the worse it hurts-I’ve been treated for depression since I was 30-I’m now 72-I have had epidurals, facets-no help, not one bit.. Have 17 % scoliosis, and muscle spasms. People I know with use stem cell-not positive.

    Have you done any videos on adult scoliosis and suggestions for pain? In 5 years my lumbar scoliosis went from Cobb angle 52 to currently 68.
    I was lucky in my 20s though. I was despondent from back and neck pain at a desk job. I transition into the ideal career: community college teaching. You stand and communicate, you pace in front of the whiteboard, you walk quickly to change classes, and have fun!
    I retired at age 64 from too much pain. I am now 71. I had a good career, but I’m shrinking and twisting (from nearly 6 ft to 5’8”). Ideas? Thank you.

    My husband was just diagnosed with this condition. Can it cause difficulty with walking? My husband cannot seem to walk normally and slides his leg instead of lifting it as you would do normally. He does tend to lay in bed instead of sitting up. We haven't started PT yet and I have scheduled him with a psychiatrist. Worried about this walking.

    I have Moderate disc height loss at l5-s1 and there is some lower lumbar facet arthropathy... will weight loss help ? Im currently doing chiropractor care and streaches also exercise at home . I hear the moderate disc height loss will cure itself with age is this true ? Any advice or exercises you might recommend?

    I also think that diet can effect how one's body heals. Yes, it's anecdotal, but before going on an oil free, whole food plant based diet, I had wounds that just would never heal completely. They would form a scab and the scab would last forever. Now that I changed my diet, I noticed that they heal significantly better and my skin looks healthier. If this is occurring on the outside, then certainly on the inside as well.
