We understand. A lot of students face this issue. First, you need to know what the issue is before seeking a solution. The issue might be that when you do not study, you are rather chill and relaxed and open for anything that comes and solves the questions with a free mind. However, when you study, you are more conscious about how to apply the concepts you studied earlier and take the stress and finally give wrong answers. Instead of using the concepts or logic, you rely more on what YOU STUDIED before the exam and simply try to replicate that.
Now that we know what is happening, we can suggest two possibilities in scenario you ar studying.
      1. You are not studying effectively. You are not making logical connections among concepts when studying. Therefore you cannot recall all those concepts/connections and thus not perform well in exams.
      2. It might be the case that you have studied well but you take too much stress while attempting the exams especially when knowing why you cannot recall what you studied. So because of that stress, the brain cannot concentrate on the questions and finally, you screw up the exam.
The solution to your issue depends on what problem are you facing.
If it is number 1, you know you have to change your study habits. Google some effective study methods and I am sure you'll find great techniques to help you out.
If it is number 2, try to calm your nerves in an exam and do some stress controlling exercises. One good way of controlling stress is doing a lot of time-bound practice of your exam at home.
      Hope this helps.
