The tree is the issue. If there is no one around to hear or be willing to listen to the issues. How much of the forest need to go down before we pay attention to the issue. Is one tree acceptable or 1000, or 100,000 before we address why the first tree fell down.
I missed this completely! Looking at a quality management system, I think more focus needs to be on employee engagement. Machines give (probably) ~95% productivity, 0 mistakes. How much gain and value is there in front line improvement through employee self worth. Make their environment a palace and ask what makes their job better. They will highlight possible improvements missed previously, and as long as constructive feedback is given. Everyone is talking and learning about the continual improvement process. It does take investment to put the headphones on and hear the tree through.
