Gentlewomen in Shakespeare's times were women of high social standing who served as attendant to the queen in this case. So she is not a maid in the way you interpret the word in the modern context even though her job is to attend to lady macbeth's needs

When lady macbeth says out damned spot, one two. According to me she isnt timing her actions. All of their deeds are playing in her head. The one two refers to the strokes of the clock WHEN IT WAS TIME TO DO THE DEED
