James's father (the older man in the video), Charles Blount, was dying of kidney failure. He only had one as he had given a kidney earlier to someone else. The immediate family was tested but found not to be matches. This video was produced and televised with an appeal for organ donations. As a result, a distant cousin in Australia, also named Charles Blount, came forward, matched, and the transplant was done. Both are doing well, though James's father of course has to follow the regimen and precautions for transplant recipients to avoid spurring organ rejection. Both James and his father are ex-military, Captain and Colonel respectively, and the line "I'm not your son, you're not my father, we're just two grown men saying goodbye' is what they would say to each other when one or the other was deployed overseas.
P.S. The family name is sctually spelled with the 'o' in it. Apparently it comes off in Britain as posh, so for musical marketing purposes, James uses 'Blunt.'
