The npc also spawns in the snow biome on an icy lake to the north west of brutal
There's actually 3 places where this npc can spawn (as far as I know)
1-the place shown in this vid
2-outside the citadel
3-outside the cave with the tilted clocktower

The reason why the npc doesnt always spawn there is because there is multiple locations he can spawn at and they are all identical to the location in the video
