Yes, you can skip those, but as replacement of bone meal you need to add rock phosphate, about half the dose of bonemeal. And instead of horn meal you can use neem cake, about twice or thrice the amount of horn meal.

Yes, you can do that. Adding mycorrhiza is always beneficial.

The final NPK might not be exact 3-1-2 or 3-4-3, but those will do the job.
