We have given xylazine, butorphanol and ketamine in the ratio of 1:2:3 ratio volume wise and then injected 1 ml for every 100 kg body weight i.v. and also inj. Lignocaine 2% at the site of incision and also over diaphragm before repair of the diaphragm rent. Pneumothorax we left as such and it ll be absorbed with the time. In buffalo and cow, mediastinum is complete and thus one side will take care of others. Thank you for watching. One more buffalo is surviving after DH which I did not upload but that buffalo is doing well. I want to convey you that DH can be done without IPPV with more than 50% success rate.

Here we need strong absorbable suture materials and we don't have thick absorbable suture materials. In DH, wastage of suture material is much more as we have to put the knot by hand tie technique. Further, silk is still the preferred suture material for closure of the ring and even closure of the laparotomy incision.
Radiography and USG are good diagnostic tool for the diagnosis of DH, sometimes only one is sufficient to diagnose and sometimes we require both thus they are complementary to each other. We diagnosed this one on the basis of radiography and clinical signs and confirmed as DH 100% on rumenotomy. Thank you 😊

This silk is used for general purpose so does not have the exact number but it is definitely equivalent to no 5 suture size.

Long term stay of Silk will not be a source of infection?
No it will be fibrosed with tissue and with time it will disappear.

We sedated the patient with a mixture of xylazine, butorphanol and ketamine anaesthesia. Also spryed 2% lignocaine over diaphragm for local analgesia
