    Thank you very much sir for sharing the video. Much grateful. I only have one question. What was the suture size of the silk used here sir?

    • @drrbkushwaha
      @drrbkushwaha  2 месяца назад +1

      This silk is used for general purpose so does not have the exact number but it is definitely equivalent to no 5 suture size.

    Great work sir, Can you explain the anaesthesia part and management of pneumothorax for this surgery?.

    • @drrbkushwaha
      @drrbkushwaha  2 месяца назад +1

      We have given xylazine, butorphanol and ketamine in the ratio of 1:2:3 ratio volume wise and then injected 1 ml for every 100 kg body weight i.v. and also inj. Lignocaine 2% at the site of incision and also over diaphragm before repair of the diaphragm rent. Pneumothorax we left as such and it ll be absorbed with the time. In buffalo and cow, mediastinum is complete and thus one side will take care of others.
      Thank you for watching.
      One more buffalo is surviving after DH which I did not upload but that buffalo is doing well. I want to convey you that DH can be done without IPPV with more than 50% success rate.

      @drrbkushwaha  2 месяца назад

      Sedation with xylazine butorphanol and ketamine in ratio of 1:2:3 volume wise and then 3 ml given per 300 kg body weight.
      Also local infiltration at the site of incision and on diaphragm before tearing adhesion and repair of hernial ring

    well done sir, but why did you use Silk instead of some absorbable suture material?

    • @drrbkushwaha
      @drrbkushwaha  2 месяца назад +1

      Here we need strong absorbable suture materials and we don't have thick absorbable suture materials. In DH, wastage of suture material is much more as we have to put the knot by hand tie technique. Further, silk is still the preferred suture material for closure of the ring and even closure of the laparotomy incision.

    Long term stay of Silk will not be a source of infection?

    • @drrbkushwaha

      No it will be fibrosed with tissue and with time it will disappear.

    Sir there is question sir why u prefer radiography than ultrasonography in this condition plz guid us sir

    • @drrbkushwaha
      @drrbkushwaha  2 месяца назад

      Radiography and USG are good diagnostic tool for the diagnosis of DH, sometimes only one is sufficient to diagnose and sometimes we require both thus they are complementary to each other. We diagnosed this one on the basis of radiography and clinical signs and confirmed as DH 100% on rumenotomy.
      Thank you 😊

    Dear Sir.
    During the suturing of the diaphragm, what kind of anaesthesia did you use?
    Did you use any general anaesthesia or just xylocaine only?
    I really like your surgeries. Learning a lot from you, dear sir.
