"2/3s of the way out". Everyone else says at the meeting point of the medial 1/3, or where the clavicle starts to move back towards the shoulder (essentially the same).

Thank you sir for your advice, but let me ask you:
In older patients with low weight the shoulder will sometimes be internally rotated so as to impede the horizontal trajectory of the needle. Do you have any recommendations? Like being more internal and more caudal?
