In the manga it's implied the burn is actually from frostbite. You can see her using ice on the burn in one panel. Essentially She realized what she had done after pouring the water on him and panicked, using her ice quirk to try and cool it down, giving him frostburns instead. It's a sad situation.

todoroki is no ordinary child, he wasn't just abused, the whole reason he even exists is because endeavor wanted the perfect quirk for his child to surpass all might, todoroki is an experiment and endeavor basically married and made his wife have so many kids, for her quirk.
A redeeming fact about endeavor since his wife doesn't completely resent him is that he probably built a relationship with her... albeit she was forced into it. All I'm saying is that prior to todorokis training she at least liked him and it was endeavors manic side that tried to surpass all-might that put a rift between them... is my interpretation from reading the manga. That's why she was sent to a mental hospital after the incident, and why Todoroki doesn't harbor any anger towards his mother and only Endeavor.

This shows Dekus character. His goal.was winning the tournament and showing the world he has arrived. But he saw todoroki thought of half his quirk as his father's and was hurting because of it and in "the heat" of the moment Deku wanted to save him from that. That instinct is the exact reason Allmight chose him and it's the true essence of a hero. Deku is interesting because he has the mindset and the power to be the strongest hero but his body cant use it yet, so he needs to figure his way out of certain situations but he won't compromise that will to save people.

To all the people saying her mother poured the water on purpose, no it really wasn’t. Later context is given and the manga shows it more clearly. His mom was becoming more and more unstable because of the abuse and in that moment, she heard someone behind her, turned, and only saw his right side peeking out, his right side which was more and more like the man who was abusing her. She LITERALLY couldn’t distinguish between him and his father. She thought he was Endeavor and, expecting abuse, reacted as such. She realized only after that it was actually Shouto behind her and quickly tried to help the burn with her ice in a panic. THAT was what caused the burn. It’s frostbite from her frantic first aid. I think All Might had the most on point take, figures as Midoriya is just like All Might :) Above all he wants to save people, winning is important but he just can't help himself and try to save others. He was one of the few who noticed Shoto's deep suffering and as the fight progressed he felt his pain more and more and eventually just couldn't bear it no more and shouted at Shoto in a desperate attempt to free him from pain. To all the people saying her mother poured the water on purpose, no it really wasn’t. Later context is given and the manga shows it more clearly. His mom was becoming more and more unstable because of the abuse and in that moment, she heard someone behind her, turned, and only saw his right side peeking out, his right side which was more and more like the man who was abusing her. She LITERALLY couldn’t distinguish between him and his father. She thought he was Endeavor and, expecting abuse, reacted as such. She realized only after that it was actually Shouto behind her and quickly tried to help the burn with her ice in a panic. THAT was what caused the burn. It’s frostbite from her frantic first aid. (Yes this is confirmed)
So yeah, it was an accident. She literally wouldn’t have done it if she had realized it was Shouto behind her, not Endeavor. She was aiming for Endeavor, not her son. She mistook him.

I think All Might had the most on point take, figures as Midoriya is just like All Might :) Above all he wants to save people, winning is important but he just can't help himself and try to save others. He was one of the few who noticed Shoto's deep suffering and as the fight progressed he felt his pain more and more and eventually just couldn't bear it no more and shouted at Shoto in a desperate attempt to free him from pain. It worked but it cost him the match

They didn’t show it in the anime, but in the Manga, when Rei poured boiling water on Shoto’s left side, she immediately regretted what she did and tried to use her ice quirk too comfort Shoto’s burn.

actually todoroki was using less than 50% of his powers while only using ice, since using both sides balances his temperature so he can use both more efficiently.

If we use physical theory, Midoriya was in the air when the power clashed. So he had less stability than Shoto that also could use his ice to reduce the knockback. I'm therms of power, Midoriya was also in a Bad situation because of the injuries he had. If he continued the battle he would eventually lose
The interesting part is the manga implied that Todoroki also launched himself into the air when the final clash happened.
