Sir in q183, the child is having episodic bleeding and jaundice since birth. But sir PNH is an acquired mutation in PICA gene. Would it be present since birth? On the other hand, haemolysis in SCD occurs when the patient is exposed to oxidative stress. Considering these two, isn't PNH a better answer? Kindly clear my doubt
Acquired defect doesn't mean it won't be present since birth.....i think it is different inherited defect in the sense that inherited defect is always present since birth but acquired defect may or may not be present since birth......
Though i don't remember it from Robin's I still gave the correct answer for ALL having the best prognosis, I remember it from the emperor of all maladies 😇

Sir, how important do you think is making notes after reading the topics from standard books, majorly for university exams, because for NEXT we have the marrow notes as making notes takes a lot of time and it is not even possible for each topic… but then seeing others make notes, I spend a lot of time in that FOMO and end up thinking even I should have made notes like others. What do you think should be done?
In question number 182, it says that condition is present since birth. So i think they are simply asking which one is an acquired condition out of these 4 options. Also jaundice is very less likely in a case of intravascular hemolysis seen in PNH. So i guess ans should be PNH right?

But due to presence of HbF for 6 months , symptoms manifest only after 6 months of life in sickle cell disease
