Yes Length of incision depends on size of pus filled uterus to be removed/exteriorized . So if needed you can cut the umbilicus, no issue. But if you go anteriorly then you have to remove or cut falciform ligament too which is attached just cranial to umbilicus.
Omentalization means attaching or wrapping of omentum around the surgically repaired area to promote healing, provide better circulation and for drainage of fluid/infected material. In this particular case of pyometra, cervical end (particularly lumen) has invisible pus/infection that is why we usually invert the ends so that infected site won;t be come directly into other organs. Secondly by doing omentalization, even if little bit infection is there it will drain by its vascular supply into the lymphatic system and ultimately into large veins i.e. vena cava. Beside this omentum also has tremendous power in tissue regeneration. This is the reason why i used to apply omentum on surgically repaired area in the abdominal cavity i.e. gastrotomy, enterotomy, cystotomy, drainage of prostatic abscess etc. Thank you for watching video and i am happy that you asked some question.

Sir, My dog had open pyometra surgery on Monday, after surgery she was doing good for next 2 days , but on 3rd day she suddenly stopped urinating, due to which her bladder got inflamed and because of that one of the stitches became loose and lite reddish fluid starting coming out. Now my vet has removed the urine from her bladder by inserting some pipe and also, tied that stich again with thread and needle, but now vet says, if this stich again becomes loose, then we have to go for the surgery again. I would like to know, can my dog bear 2 surgeries within 1 week ?
In surgery, complications may occur at any time depending on how surgery has been performed? and skill of surgeon. Moreover only doer/surgeon can tell why this happened? Variations are always there in biological science. Whether 2 surgeries c an be done or not that depends upon the condition of the patient.

Ovary should not be left while doing spaying or pyometra surgery it may cause stump pyometra in the lator stage of life

Sir, Do you have any idea why did the dog suddenly stopped urinating after 2 days, what can be the reason behind it ?
