It is likely that Miis don't die from falling. For example, in the trampoline minigame on Wii Fit U, where you have to jump as high as possible by jumping at the right time, if you jump VERY high and then completely miss the trampoline your Mii still survives without any (critical) damage.
this game taught me the term "dead meat" I'm not even kidding, I remember it clear as day, granny was watching me play (I think I was 4-5) and I fell and she said you are dead meat! I laughed and said YUM! lol looking back she must've been laughing so hard internally

The difficulty of the games is very inconsistent in Wii Fit. In some of the games, you can get 2 or even 3 stars playing it for the first time. Balance bubble and tightrope tension ends as soon as you lose balance and will not give you more than one star unless you reach the very end by doing everything perfectly. They require lots of practice to even get 2 stars on beginner difficulty.
When I was little I removed playing wii sports and I remover all the sad music and the fro or people and a celebration every time I won and some how my cousin either bear or almost beat the rope one and jumped over that thingy
I think Nintendo removed this game off of Wii Fit Plus since it showed and referred to a person that can be suicidal and showed an action that people would attempt and then kill themselves by trying to do it.
