The same thing happens with a normal demoman. If the kick misses then the next 2 hits do more damage. It's because attacks in a combo deal reduced damage, except for the first hit. If the first hit doesn't deal that much damage on its own, then then the next hit will deal reduced damage, and in the case of Paul's demoman and heat smash, the damage lost from second hit is bigger than the damage from the kick.
