@@KitsuneFyora It happens regardless, although you get a bit more dialogue during the attempted bite scene if you don't finish the repairs. There is a way to avoid this outcome, however - If you don't long rest at all after defeating Cazador, and you finish the entire game on the same day, there will be no time for this to happen, and Ascended Astarion has a unique scene where he tries to convince Karlach to go back to Avernus with him (albeit in an annoyingly condescending way).
Ascended Astarion is always an evil choice, but especially here.
