When I've seen oxygen levels as high as that it usually indicates that there is no fuel entering a cylinder. The CO and HC levels confirm this. The cylinder without fuel is just pumping fresh air into the exhaust system. Now that doesn't necessarily mean the fuel system is at fault as an ECU can shut the injector off even if an ignition misfire is the cause. This is to protect the catalytic converter from unburnt fuel.
