    This is the story of Pakistan international airlines flight 661. On the 7th of december 2016 an ATR 42-500 was flying from chitral airport to islamabad in pakistan. Todays flight would have one captain and two first officers and 42 passengers on board. One of the first officers was being trained on this route. The plane took off from Chitral at 10:38 am the first officer was the one at the controls. The plane climbed albeit a bit slowly the Atr 42 usually climbed at 160 knots but today it was lagging a bit behind at 145 knots. At 10:45 am the plane levelled off at 13,500 feet and the crew and the passengers settled in for the journey. As the plane cruised the captain took over flying duties from the first officer.
    As the plane cruised at 186 knots at 13,500 feet the captain started to make an announcement at 11:04 am, he was interrupted by a warning, A pec or propeller electronic control- 1 warning for the left hand engine appeared. The first officer lets the captain know and the fault was acknowledged by the captain. The captain called for the QRH or the quick reference handbook and summoned an engineer who was a passenger on this flight as the pilots did this they also brought back power on the left hand engine the plane slowed down to 146 knots. They were trying to reset the Propeller electronic controller, they much like us hoped that turning it off and then turning it back on again would solve the issue. But it didn't the captain wanted to do the checklist to reset the PEC again. They tried resetting the PEC 3 times with no luck. By 11:10 am the engineer joined the discussion in the cockpit. The right hand engine was holding steady at 82% but the left hand engine was all over the place. At first it was around 82% then it dropped to 62% then it climbed to 69%. It was as if the left hand engine had a mind of its own.
    At 11:10 am a sound was heard from the left hand engine and torque from the left hand engine dropped to 0. The left hand engine had failed. The captain said “engine gone”, the pilots carried out the single engine operating procedure and as a part of that the captain pulled back power on both engines. The propeller speed of engine number 1 or NP-1 was quite high it was at 102%, this caused some asymmetric drag on the left side of the plane, sure when you lose an engine you expect the broken engine to produce some drag but the drag by this engine was well beyond what anyone had expected, three times as much to be precise, the autopilot held the plane.. For now. The condition lever was moved to the fuel shut off position. The propeller speed started to drop and it was down to less than 30%, the captain increased power on the left hand engine. At 11:11 am The crew wanted a lower altitude and they put out a mayday call. The propeller speed for engine number one was at 25% as expected. But it didnt stay there. Soon it started to rise soon it was at about 125%, the rise in the speed of the number one propeller went unnoticed, they only noticed it when they picked on the noise from engine number 1. Soon after that the autopilot disengaged. The captain struggled to keep the plane under control. The plane at this point was close to a stall, The speed had dropped to 127 knots and they were descending. The plane turned left as it lost altitude and airspeed. The captain was trying his best to turn the plane to the right but it was stuck in this left turn. The stall warning came on again and the stick shaker activated. The crew were confused; they tried to feather the left engine and maybe they even attempted a restart. The power on engine number 2 was increased and the captain commanded right rudder right as the speed on the engine one propeller dropped. Without the drag from the left engine forcing them into a left hand turn the plane was now in a right turn and the plane stalled. The plane was uncontrollable at this point

    I used to be an Aircraft Mechanic and I had a powerplant licence. I remember the icing issues that the ATR 42 suffered and was happy when they were resolved. This was an accident that was just waiting to happen. Faulty maintenance has cost many lives.

      And that’s why it is important to follow the correct maintenance procedures as well as troubleshoot so the aircraft can be maintained properly and the defects can be eradicated

      ohhhhh laddie I was there. It was back in the days of the Queen mum when she gave all the airplanes to the children. we were in that plane. we ate the lefthand engine and had some asometric drag for dessert. It's ok my lad. the stick shaker.

    It was criminal negligence on part of PIA to allow that plane to flight, multiple faults had already been detected in that model of aircraft and yet they continued to operate it. Political appointments to the highest order in PIA like every other public institution in Pakistan have made quality procedures a thing of the past and are no longer actively pursued or held up

    I must say, I really appreciate your humility in asking those who work with the planes in question to correct any errors, so you don't make them again going forward . That is the essence of crash investigation as well, of course, making things better by learning from mistakes, and failure points. Just so refreshing when some channels might say eff off to see someone so open to constructive criticism.

    PEC fault on a ATR in fairly common, usually on the ground the reset does the job usually after start sequence due to small micro circuit cuts on the aircraft.
    I have heard of PEC failing in flight too but it’s manageable manually, in this case the plane was pretty much domed due to the various other problems it had which could not have been controlled from the flight deck. The prop not feathering produces so much drag on Pratt Witney engines that it won’t let the ATR plane climb even at lower altitudes. Flying this in the simulator with engine failure and without Auto Feather, it’s a very big deal as it’s close to impossible for the plane to climb and control. There is whole Emergency briefing that dictates manual feathering of the engine before anything else can be carried out otherwise the plane will bank, yaw uncontrollably, loose all the energy, stall and fall!!

      You seem to be in the know. Is the propeller motor hydraulic since the OSG seem to bleed off oil to return?
      If the blades seized in 0 pitch (100 percent drag, 0 percent thrust), that makes the plane unflyable from what I understand. Can't you just install some form explosive to sever the blade hub from the engine should this situation occur? Hydraulics is incredibly sensitive to any debris so there are many things that could potentially lock the pitch of the blades.

      Yes the props move via hydraulic pressure and counter weights on the prop root from windmilling to feathered, only pilots who fly on the line know how serious a unfeathered prop can be, it’s like have a parachute on the back in the aircaft

      is a controlled 'crash' possible in an ATR, without feathering the inop engine? Ie. a better situation that just slamming into a mountainside (somewhat controlled landing on the valley floor).

    Your presentation is very good. As an ATR instructor with an interest in aviation safety, I have read the accident report. The cause of this accident is very complex; the odds of getting three failures in the same engine is incredibly small. You managed to explain it well.

      A question for you pls; why did the pilot/captain not abort the flight during the incident early and return back to the airport it took off from? Why continue on his flight plan?

      @@4evertrue830 Hi, from what I understood, the plane was halfway from either airport, The plane has to go over Hindu kush range at Lawari with peaks over 12,000ft. The plane had already crossed the range and was going over the foothills. The plane was having trouble climbing it wouldn't have made over the range. Had it crossed the last mountain where it crashed, it would have entered a flat plain land.
      Besides, Islamabad Airport is an international airport with all the facilities, while Chitral airport is a small seasonal airport with nothing, not even a refueling station.

      @@patjohn775 Nooooooo!!!! Not TV!!! Lol. Somewhat dramatic, I know. But seriously. Mini does such a good job of researching these events and providing facts of the incident and causes in a non biased very concise presentation. He breaks everything down in a manner that anyone can understand even if they are not a pilot and don’t understand how airplanes work. I’m concerned that producers would ruin his work and shift the focus to the “drama” and butcher (they call it editing) his presentations somehow into a reality TV abomination and hour long, but somehow have less content the way they dilute any show with a modicum of actual tangible information in them. I do agree that he needs more compensation and recognition for the sheer amount of time and effort invested in his excellent work. Could you imagine what the history channel would do to something this good? They would attribute everything without a clear cut causation to fucking aliens!!

      @@matthewl67 Yes! Spot on, history channel and National Geographics also, claiming sea monsters as the cause of missing airplanes in the Bermuda Triangle. ( LOL) Rather than a lack of landmarks, as everything looks the same over an ocean. A deviation in unexpected IMC with unknown winds and unknown wind correction angle, whilst navigating by dead reckoning over an ocean, was a definite way to get lost and run out of fuel. Back then there were no Gps or meteorological satellites to determine winds aloft or surface winds over the ocean, which means an attempted rescue would be searching in a completely different area and would also risk getting lost. Add to all of this, compass errors and magnetic deviations, as flying by a compass would have been the only means of direction finding over an open ocean.

    A near relative of mine was the first officer. I remember the day like yesterday when I first heard the news on TV that an aircraft coming from Chitral had crashed and I immediately thought about my relative hoping he wasn't on this flight as they only showed the captain's name and picture and not of the first officer. Few minutes later I got the messages from other family members confirming he was on this ill-fated flight.

    I very recently discovered your channel and subscribed. What is so very appealing is the narrative of just the facts. I'm not saying lives don't matter but I like hearing what happened and what went wrong without the time consuming sensationalism of the human aspect.

    Blades were recommended to be replaced at 10k hours. This failed at 10,094. Do manufacturers leave their guidelines so close to the point of failure?
    I would have thought they’d err on the side of caution and recommend replacement at least a thousand or so hours before they’re expected to fail.

      That must have been a very premature failure (probably due to poor maintenance on the part of the airline) as there's no way any engineer worth his salt would leave a factor of safety that tiny, it's not even 1%!

    I have 25 years of public/civil service under my belt. This started off as one of the most complete investigations/reports I have ever heard/read.
    Having said that, I would like to see/hear about all maintenance requests and/or pilot complaints for maybe, five years leading up to the incident (if more or earlier maintenance requests were available, I would probably peruse them as well). Names of everyone having done maintenance during those said dates; and the disposition/changes made, to either parts or protocol that occurred in the aftermath and subsequent investigation(s)and official report(s).
    This video definitely deserves a follow-up.
    Maybe interviews with pertinent witnesses.
    Hour-long documentary? CVR and ATC recordings?
    Great work, i am subscribing ringing the bell.

    Moral of the story is Don’t fly on Airlines that don’t maintain their planes.
    It’s sad that PIA is not a cheap airline but still they try to cut corners and don’t invest in buying new planes or at least maintain their planes properly.
    TSB’s are very important in Aviation.

      How do you know if an airline does not service their fleet regularly? Is their a site online or a bulletin that provides such info to anyone worldwide? Just asking.

    There seems to be an inerrant problem with the ATR 42 & 72 engines, numerous reports of engine problems over the years have led to many incidents. I would not fly on any regional Turbo prop aircraft especially in countries where money is not available for proper maintenance. Back in 2006 I was in Iran waiting for a flight from Mashhad to the old Tehran International known as Maher Abad, our flight was to be on a TU154 there was a problem with one of the engines that they were trying to fix. Four hours went by when they announced boarding, I refused to board the plane as I had a bad feeling about the plane. Iran Air put us on a Fokker 100. Anyway a few days after our return to the UK I heard that the TU154 we were due to fly on had crashed near Shiraz.

    Very well presented video and supporting simulation. I can only add that compromise on safety standards and lack of professional attitude led to that tragedy which could have been avoided. Pia engineering is one of the oldest and largest in the region yet they failed to implement the safety standards in its letter and spirit. I really hope that they have started addressing such issues as they just passed the IOSA safety audit

    When the OSG failed it affected the PVM and it's EHV, which flummoxed the PIC and PNF until the plane was SOL.

    I think a very similar failure happened in the USA. I just remember that the feathering mechanism of the prop failed and the plane was uncontollable. A former NASA astronaut was amongst the passengers. They all perished.

    The problem i see with this flight is more about pilot pressure to complete the flight regardless of circumstances. On climb out, they only reached a max speed of 145 knots; just 25 knots over their minimum? That's the point I would have started looking for a landing site and getting that plane on the ground safely. Then you add the fact they didn't feather and shut down that engine at the first sign of a pitch alarm? That's problematic. Let alone the fact that they were allowing autopilot to fly the plane when there were obvious mechanical problems that the autopilot may not have been able to account for.
    In this situation the saying, "Its better to be alive and embarrassed, then dead."
    And by that, I mean, at any time after seeing just that 25 knot window to the planes stall speed they could have turned around.. but chose to continue.

      I would just like to add the following;
      Oftentimes in these situations there are "other unknown" pre-existing maintenance issues which the airline will try to hide either from accident investigators or from civil lawsuit attorneys, but was a serious issue for the pilots. Yet it never gets to the knowledge of the general public. 2) It is so easy to armchair quarterback in the comfort of a quiet room. Where there are no blaring bells and loud sounds, and no impending fear of doom. In that emergency situation where you don't have control of the aircraft and you are literally fighting for your life, you need extra time to think of something new, when what you have been trained to do is not working or correcting the situation.

    Would any of you say that they might've been able to land somewhat safely had they not been in mountainous terrain?
    I feel like they might've stood a slightly bigger chance if the area surrounding them had been flat, but sadly, the odds were really against them. :(
    I ask because I'm not really an expert. What I've come to know about aviation is all thanks to videos like these!

    The lack of maintenance oversight is typical of PIA and other carriers with little government mandated requirements for suggested repairs from the manufacturer. Also you can see that these were airplane operators not pilots.. (Typical of PIA. See the PIA crash 8303. Failing to put the landing gear down???) If they flew by AQP standards they would not have stalled the plane. They would have attempted to put the plane down. Great job in describing this event!

    Very good, except the continued display of animated flight segments that are totally unrelated to the accident with the recitation of investigation results are very distracting. You interspersed diagrams of failed parts (relevant) with a plane bouncing on the runway during landing (this plane crashed into a mountain). Don't use filler like that - it detracts from the product.

    I always wonder, how you can read what has happened from the parts left over after a crash.
    how do you know what's impact damage and what was broken before the crash?
    I can't even tell you in which way these investigators are amazing, because I can't comprehend how this works :-P

      You can actually tell if something broke off during flight or if it's impact damage there is some great documentaries here on RUclips explaining how that's done.
