Kakashi probably omitted reporting having fought the demon brothers, Zabuza and Haku so his team wouldn't be reprimanded for not reporting back after they found out the mission details were false and also to protect old man Tazuna's standing as a client to the leaf village. Therefore, the mission remained as a C rank mission.
Agree with @swordsmanoftime3001 I would also necessarily add that these cards are not some magical thing that will find out the truth about the user. They are just cards that have secretly written information on them that can only be read by the chakra of the person who placed them. So the information may be incomplete and inaccurate, depending on what the person has found out.

Neji was right about all the Hokage being related or teaching each other (and will be for literally every Hokage who wears the hat even up to Boruto) but the 4th has no exceptional lineage, much less being a Sarutobi.

If you're referring to the bottom-middle chick, I think she takes notes on different topic points she wants to bring up for her stream after the episode ends. This way she doesn't talk through the whole episode or end up stopping the episode a dozen times.

Also, they took this trump card and literally turned it into a plaything for one dude just so they could have a wave of exceptional folks - along with bringing back some favorites. This version was more... I was gonna say realistic, but I think believable fits better. Like since this was before all the ridiculous DBZ-styled power ups, it was actually interesting to see the Third take on the first two Hokage like this. Meanwhile, much later they overpowered the fuck outta everyone so it actually kinda makes it less impressive.
