Mam, does that mean from the graph is that if 1 month have been passed after the day they decided to take the 1st dose of pep,one can safely assume that they are saved from that possible exposure as enough antibodies would be developed before rabies enters nerves?

If a person presents to us on day 12 telling that he /she took 0 and 3 doses but missed day 7 , we will continue vaccine as you said. That means on the presenting day we give 3rd dose. Here when will be the 4 TH dose? Is it after 21 days from the presenting day or 21 days from the actual 3rd dose day?
Yes 21 days from third dose... Keep interval between doses fixed as much as possible

Ma'm, First of all, you answered all the common doubts, also thanks for creating awareness about Rabies.
At 11:10, you mentioned immunoglobulin may be needed even in Previously vaccinated person in category 3 bites. I read 'Anamnestic response of antibodies after booster doses that even category 3 bites also neutralised before rabies reaches brain' ? Aren't booster induced antibodies fast enough to overtake rabies virus and neutralise it? So why does a PreP immunised person like me, still have to go through all the troubles and pain of getting my every bite-site infiltrated with Immunoglobulin and Full PEP course ?
Sorry if I confused you.. At 10.00 it's stated if you have taken a complete pre or post exposure only 0,3 needed no immunoglobulin.on reexposure..
The next scenario is if someone unnecessarily gave immunoglobulin.. Then they should not limit to 0,3...

thanks for the quick reply Ma'm. You replied 'if someone UNNECESSARILY' , that clarifies my doubt. So I am safe to assume that just 0&3 booster doses enough for previously vaccinated (pre or post), immunocompetent person even if its category 3 face bites.

Govt should make awareness of Pre exposure vaccination. So even immunoglobulin not available, vaccine alone can save lives.

Ma'am,I am very tensed , will really appreciate if you can reply. A rabid dog was found and taken in captive from the IIT Bombay campus on 16th of March. We visited the campus on 24th March, where my three year old daughter fell down on the street and got a minor scratch on her knee which was covered with her pant. Now is there a possibility that she can get rabies from the virus that could have been on the road surface from the drooling of the animal or any other animal that it might have butten?
No it is not possible...rabies virus is very labile...it cannot survive on a road surface .once material containg the virus dries it is not more 8nfwctious
Hello Doc, wonderful presentation. I took full course of ARV (0, 3, 7 and 28th day) because I was unsure if I was bitten or not. Can it be considered Pre-exposure prophylaxis? Also, the first three doses were IM and the fourth dose was ID on both arms. Will I have better immune response now or was it an overkill?
What is IM ..where they will put ?
For my son they put 4 injections in both hands...in Govt hospital..I don't know it is IM or ID
IM means intramuscular and ID is intradermal. Both injections will be in both arms. There’s no difference between the sites but the route of administration.
Ma'am after taking 2nd booster of rabies vaccine there is minute oozing of blood coming out of injection site after needle is removed out..is there possibility that vaccine will also come out along with small amount of blood ??? I have taken intramuscular
No small amount of blood oozing will not decrease vaccine efficacy

In case of any future exposures if you are immunocompetent you need to take 2 doses... day 0,3....booster doses produce good anamnestic response...a time limit is not specified...

Only vaccine needed the person already has memory cells it will immediately respond and produce antibody..

Mam,kindly give your opinion-A previously fully anti rabies immunised person who has taken 0,3 Id for 2nd exposure after 4 months of full immunisation, does he require vaccination(0,3) for a 3rd bite 1 month after the 0,3 dose(2nd dose)?
After 2nd 0,3 he is protected for a minimum of 3 months... Within that period vaccination usually not needed.

Mam i have taken full course of rabbies vaccine when dog bite me ie (0, 3, 7 ,28)days, but after 4 month 15 days i again came contact with street dog so i go for the vaccination in government hospital doctor said you need 3 doses ie (0, 14, 28) but i am confused you said only 2 doses is required if we have taken vaccine earlier ie (0 , 3)day what should I do know should I go with( 0,14 ,28)day or should I talk to him because today is the 0 day i have taken a dose and the vaccine is given on both the shoulders ie (half one side and half another side) please help mam it's urgent 🙏🙏🙏🙏🙏🙏🙏🙏🙏🙏🙏🙏🙏🙏🙏🙏

Ma'am our phc nurse has given more than 0.1ml intradermal dose on both site to me on day 7. What will happen ma'am? I think she confused with the marking. It is a 40unit marking insulin syringe and she has taken 10unit and injected on both side with two 10unit. Please reply ma'am.
Don't worry nothing will happen .. Cumulative it will be equivalent maximum to a single im dose... Mostly half dose... If it was 0.5ml vial

The insulin syringe was 1ml volume 40 unit marking and vaccine was abhayrab. She has taken 10marking which equivalent to 0.25 ml on both side. It's on day 7 but day 0 and day 3 was given correctly.
Ma'm, PReP second dose vaccinated on day 8 instead of day 7 for my 3.6 yrs old kid ? Is it ok ?

Hello doctor. I got dog bite on hand above thump finger on september 8 2022 a few drops of blood came out. I had washed with water and hand perfume, as no other things were available. I took one injection of rabivax s 1 ml vaccine on same day.
And another shot of rabivax s 1 ml vaccine on october 7 after 30 days.
I had took only two shots and I don't know about that street dog whether it is alive or dead. The wound has been healed in 5 days only there is no itching. I Am worried will I get rabies please suggest what should I do now?....Shel i take vaccine course 0, 3, 7, 28 or what should i do now
Today 65 days have been completed, after dog bite.

Take the third dose today.. Followed by fourth one week later and fifth dose two weeks after that. That is no need to restart start from where stopped.

Respected madam, one of my friend got a scratch by a bat on her forehead during day time(In Odisha). Does she need vaccination?
Exposure to bats is taken as an indication for anti rabies vaccine in many countries... But bat rabies has not been reported from India and hence usually not considered as an indication.. .. but getting bitten in the day time is unusual... and nimhans study had reported rabies antibidies in bats..though no bat rabies cases have been reported so far...would say may take anti rabies vaccine..as. in fact any opportunity to receive vaccine take it as rabies death is quiet common in India

Hi doctor mera pura vaccination hua hai rabivax-s intradermal injection ka 4doses (0, 3,7 and 28 days) in December 2020 and not sure i was provided with rabies immunoglobulin that time doctor injected something around my scar. Abhi mujhe 17 th October 2022 mein puppy ne tea ko lick kia jo meine galti se pi lia. Hello mam, Me and wife got bite from monkey Category 3 , Day 0 (September 1) and day 3 we got Vaccine but on Gluteus Medius region , did Vaccine become invalid (not given in deltoid region)?
And We went to Dr RML hospital Delhi on sat Day 3 , They did skin test for equine (Horse rig ) they found both of us reactive , They ask us to come on monday (tomorrow) to get Human RIG (Day 5) , will it become too late to get H-RIG ?
Yes vaccine in gluteal region is invalid needs to be repeated...
Rig can be given upto 7 days of starting vaccination... Sooner the better..

No you will not get rabies like that... Rabies virus is very labile it cannot survive outside body for long.

Do normal cows, buffalo and goats transmit rabies if they have no symptoms?
If bitten by a rabid dog they can transmit... But in general rabies appears in saliva usually in symptomatic animals.and once virus in saliva unlikely to survive beyond 10 days...
But remember most studies are done in cats and dogs...other cattles duration can vary. ... When in doubt vaccinate...

Madam.. One doubt.. After what time can a patient donate blood, after taking ARV, following cat scratch, (cat2).
Disease as such is usually not transmitted by blood and vaccine is killed vaccine.. Rabies can be transmitted from human to human by organ donation especially cornea.
Hence better to donate blood after vaccination over

Is rabies virus transmitted through sneezing and coughing from rapid animals?
Rabies can be transmitted if secretions from rabid animal falls on a wound or on your mucus membrane like eyes or mouth... As such rabies virus is very labile it cannot survive in environment for long... It otherwise does not spread through aerosol

There is no upper time limit.. WHO guidelines says even after 10 years 0,3 schedule gives a very good booster response as memory cells are also there

Hlo mam i took 3 doses of id vaccine on government hospital 0,3,7 no 28th day and after 1 week of my 3rd dose final vaccine i had a 3 deep bite marks on my leg i think its a bat bite but i didn't wash it till 4 days but after washing it the healing part came out i don't think a insects could do that deep bite what shall i do please help me🙏

Once dog symptomatic and rabies virus present in saliva dog does not survive for more than 10 days... Mean survival is 4 days... So if dog is still alive don't worry

Good information but what if virus enter in nerves and become dormant or slow mover and reach brain after 6 years that the question i think eveybody now a day looking for because they had dog bite expoure some day in their life and they think virus might have gone inside the nerve and vaccine will not do anything now on nerve so wait fke death now by rabies. People are scared by remembering their dog bite in childhood and today they are 40 years old but scared thinking that virus might reach brain any time
Can rabies be spread by buying, preparing, and eating goat meat ?
Once cooked rabies virus cannot survive so cannot cause rabies... But eating of rabid meat should be avoided.

Mam if dog alive after 20 days we need rabies vaccine
Once dog gets rabies and is symptomatic it is unlikely to survive beyond 7 days cdc says cases of rabies after 10 day observation period has not been reported... But once bitten never wait for 10 days always start vaccination and observe dog.. If dog ok take day 28 vaccine and make it pre exposure prophylaxis.

If a dog is alive after 10 days we are already taking 3 doses on 0, 3, 7 and again as you said we need to take on day 28...so how are we converting it to a pre? As we are taking all 4 doses hence it is a PEP right?
Yes if you are right if you are on id regime it will remain pep....infact who doesn't recommend day 28 for pre ep but since 8ndian government guidelines recommend it ...it is better to take...otherwise also a dose distant from the initial dose gives a higher ab titer
