They need to update the drowned thing. Idk who walks into a bathroom finds a dude soaked in puke and toilet water and thinks to themselves "oh man what were you thinking? That toilet is clearly to small to swim in..."
To be fair, I imagine if someone passes out while throwing up in a toilet. It'd be possible to drown. In hitman they're probably going off that. Still goofy considering they splash around and all.

I like to think that 47 killed so many random people in toilet that it became a new ''Rare Way To Die"

I think the idea is more that it looks like the person may have overdosed or had some other medical issue where they vomited into the toilet until they succumbed to the issue

The difficulty of the challenge is very hard to be felt this way. I think you should add some fails, and also give the tenser moments some more time on screen.
agreed, like thebones5, he is a fnaf youtuber who also does difficult challenges, but he adds fails and shows his suffering, it makes the challenge look 10x better
