The charcoal filter is to prevent others near you from breathing it in.. like the paramedics that are in a sealed box with you. But outdoors and no one near it wouldn't be necessary at all. Even an open area even without a slight breeze should be sufficient as to not necessitate use of the filter. Even in a small hospital room I doubt the staff will worry at all if your not using the filter..
