The apical pulse can be heard at Erb's point, 3rd intercostal space left sternal border, or at the 5th intercostal space on the midclavicular line, probably directly under the left nipple. If one assesses the radial pulse you might try counting the pulse for the first thirty seconds, multiply that by 2 if the pulse is regular, and then start to count breaths without moving your hand from the radial pulse for the remaining 30 seconds so as to avoid causing the patient to alter their breathing patterns by drawing attention to them.
