At 36:31 Most people chose to stop ceftriaxone and start mero for the ESBL producing E Coli, and you said that is the correct answer. But ertapenem is also an option and has a narrower spectrum, so with antimicrobial stewardship in mind, that looks like the best choice to me.

Excellent webinar. I am learning lot from your lectures.
Regarding the meropenam continuous infusion, I think there is a problem with stability. Hence, they recommend 3 to 4 hour infusion. We should not extend the infusion more than 4 hours.

On the Q&A section question about pip/tazo and ESBL- The IDSA guidance on using pip/tazo ESBL is a somewhat pragmatic one.
They carve out an acceptable role when piperacillin/tazo is started empirically, but subsequently Cx demonstrate or imply ESBL production. This is for uncomplicated urinary tract infection, with with patient having clinical improvement. This recommendation is not a step down option, which in the video discussion was not clear.
For pyelo, pip/tazo is not preferred (carbapenems preferred, with FQs and tmp/sulfa preferred oral options if suceptible) and the IDSA panel expressed concerns, but didn't explicitly recommend against.
But for outside the urinary tract explicitly recommends against its use.
