A doujinshi is a fanmade manga made apart from the real series, which means it's based off the author of the Doujinshi's view whether or not to make the characters gay, straight or lesbian. In this doujinshi, Izaya and Shizuo are "Gay" for each other. Izaya doesn't usually look that feminine, it's just the artist's way of drawing. In the manga, Shizuo aren't outright gay for eachother.
You want R-18. You know, if you're not fussed whether it's doujin or actual manga, you could just go to any manga site and search for "yaoi" and "smut." Honestly, it's really really easy to find lists of the stuff...*cough cough* Or so I have heard.
