The guy that did the background screams is not actually from the choir. That guy is Jordan Fish, a member of the band, but I understand why you thought that it was a guy from the choir :)

Its about heroin and the addiction he used to have to it. Like when he says this has got the best of me and I can't seem to sleep, is because when going through withdrawals it keeps you from falling asleep as well as extremely sick.
