The best way to save a choking victim is no longer 'the Heimlich'

  • Many people believe the "Heimlich maneuver" is the only viable option to help save a choking victim. Turns out, that's not really the case. Following is a transcript of the video.
    What's the right way to save a choking victim's life? It turns out, the Heimlich maneuver is not the only approach - and it may not even be the best one.
    Repeated blows to the back could be equally useful in a dangerous situation. You might be thinking that back blows will only lodge the food deeper into a person's trachea. But this is a myth perpetuated by Dr. Henry Heimlich.
    According to reports from Dr. Heimlich's youngest son, Peter Heimlich, the founder of the Heimlich maneuver spent years trying to discredit back blows, publicly denouncing them as "death blows."
    He even funded a study in the '80s that showed back blows could do more harm to a choking victim than good. But in truth, there is no valid scientific evidence to prove that back blows are any better, or worse, than the Heimlich maneuver. Needless to say, Dr. Heimlich’s questionable actions led the American Red Cross to make a big change to its first-aid protocols in 2006.
    Up to then, the Red Cross had - for 20 years - only recommended the Heimlich for choking victims.
    But in 2006, it made two big changes:
    First, it removed "Heimlich" from the name "Heimlich maneuver" and relabeled the method as "abdominal thrusts."
    Second, it changed its protocols so that the recommended way to save a choking victim's life is now a 2-step process:
    Step 1: Administer 5 blows to the back by hitting the palm of your hand against the area between the shoulder blades.
    If Step 1 does not fix the problem, move on to Step 2:
    Perform 5 abdominal thrusts by first placing your fist around someone's stomach with your thumb against the middle of the abdomen - above the naval. Then, wrap your other hand around the fist and thrust upward.
    If the food is still stuck, repeat the process starting with Step 1.
    If the victim falls unconscious, start performing chest compressions with rescue breaths.
    First, lift the chin and tilt the head to open the airway. Pinch the nose shut. Make a complete seal over the person's mouth and blow air for about 1 second. Perform 30 chest compressions, pushing hard and fast in the middle of the chest. Look for and remove objects in the airway.
    If breaths don't make the chest rise, repeat the process.
    You should also consider calling 911, and do so immediately if the person passes out from lack of oxygen.
    Time is of the essence. Act fast, and you may just save a life.

    It's also good to know that many people who are choking will just look like a deer in headlights and be unable to speak. That's why it's always good to ask someone if they are choking. And if you see someone standing up in a restaurant for no reason staring off into space and looking terrified, they could be choking.

      Yeah, thanks for pointing this out! I swear, this one time I was choking while one of my friends was over and I came upstairs to try and get help, and she had no idea I was choking! In fact I think she even made a joke when I came through the door, not realizing the situation. Ugh, it was so scary and I thought I was gonna puke. Thankfully I didn't, I managed to somehow swallow the offending object (in this case a pill) but still. Suffice to say, afterwards I was pissed at her for not helping lol.

    this video is wrong about some things...
    1. start with compressions
    2. do 2 breaths not 1
    3. do a "head tilt chin lift" if the breath does not go in but don't do the breath again unless you still have 1 to go.(the life is tilting the head back then forward to the same position to try to dislodge the object)
    4. don't to too big breaths or the person will throw up (with stomach acid which will burn their throat)
    5. don't start the back blows or abdominal thrusts unless they are not coughing and have a full blockage
    also they renamed the Heimlich because the guy wanted money for the red cross to use his name.

      Thomas Adriaanse the information is from American Red Cross so it can't be too inaccurate. Also it said "in 2006" so maybe the process changed since then?

      Agreed. Some other tips that may be helpful. Some are reluctant to do the breaths - especially if the victim is a drug user, have vomited or similar. Two things that may help (I know this *is* a compromise): #1 Ask the said drug users friend (if such is nearby) to do the breaths on your command (the friend is usually less reluctant) - typically if your helping somebody who's a "junkie" and lies lifeless on a street corner. #2 Alternatively place a handkerchief, scarf or similar thin air permeable cloth between the victims mouth and yours. If people have these two ideas in the back of their minds they are more likely to help.

      When I do the breathing part on the person, am I supposed to blow IN air or suck the air OUT of the person(like a breath in for me)? I exhale carbondioxide and the person needs oxygen so why would it be good to blow my exhaled air into them?

    OK, I saved my middle son from choking on a hot dog by doing the Heimlich when he was 9, so I'll just go with that! When I was in the Air Force, one of my extra duties was to teach my entire squadron self-aid and buddy care (treating for shock, setting broken bones, fireman's carry, Heimlich, etc.). I did this every month for 3 years. When my son began to choke (he really did put his hands criss cross on his throat like they trained us). Even though it was 14 years since I'd taught the Heimlich, I grabbed my son, spun him around, gave 2 thrusts, and it was out. Training is everything. If you have young kids especially, it's so important to learn and practice.

    I was taught the heimlich in elementary school, i have used it several times from the age of 10 on. It works! Teach it to your kids! I have saved my own children and a younger brother with this!

      Actually these information miss something really important :
      - Before using either back blows or abdominal thrusts, you first need to confirm to the person she is really choking (means she can't breath : no talking, no coughing, you can even ask "are you choking" or something like this to confirm). Either abdominal thrusts or back blow can worsen the situation if the person is still able to breath.
      - abdominal thrusts are a bit more complex movement : compressing then going upward, but even if you are not doing it perfectly right, they are still effective.
      - Best way to learn these two methods is to discuss and learn them from rescuers or medics directly, so they can show you.
      Please note I am not a medic nor rescuer, so don't trust me 100%, just go to see rescuer or medic to learn from them, they will be much much more accurate than me or a video on the internet.

    I rescued someone with the Heimlich last year. One of my fellow cooks, who was always stuffing food into her face, choked on a piece of steak. Two thrust and it came flying out. The Heimlich worked like a charm. She, on the other hand, didn't learn her lesson and continued jamming random food in her mouth and talking at the same time.

      The (reasonable in my opinion) theory goes that it's because humans are not supposed to eat meat. Statistics of deaths by food suffocation seem to confirm this. (Apart from being careless while eating, of course... But that's another issue.)

      If a fish is not cleared from the bones completely i never eat them i kinda have fobia from bones and no i dont lack the omega 3 because i eat chia seeds which have more omega 3 than any fish maybe except macrel, which has a looooot of threatening tinny bones

    We were taught this procedure in 1984 and in 1990 when I did a refresher. The only essential difference being the number of breaths against the number of compressions. Back-slap followed by thrusts ......

    Always ask the casualty if they are choking. it sounds obvious, but if they can respond verbally (eg. saying "yes"), their choking is minor, so back blows should dislodge the obstruction. If the casualty can only respond physically (eg. shaking head), then their choking is major, so use the Heimlich maneuver (also known as "abdominal thrusts").
    For reference, i am a qualified First aider (i hold a First Aid at Work qualification).

      Most of this is correct, except if the choking is mild you aren't supposed to do back blows - you are supposed to encourage them to cough, until they either cough up the food or go into severe choking (I am also certified in first aid)

    I've also used the heimlich twice and weirdly in both scenarios the food came flying out across the table on the 3rd or 4th thrust, its absolutley is efficacious, not to take away from back slaps but obviously back slaps first especially if it is a partial obstruction but if it is a full obstruction and they cant talk or cough I'm leading with the heimlich it's worked twice so far lol

    I was at a resort once and saved a lady's life who choked on the all-inclusive steak...abdominal thrusts worked well in that occasion, thankfully...great to learn something here. She was 80ish y.o. and on her 50th anniversary. Chew well people....or buy better steak :)

    When performing CPR continued chest compressions are what's key not breathing into the airway. Because keeping the heart pumping is what will ultimately save someone's life. Once certain a person is no longer breather and no pulse immediately begin compressions, take turns switching out with someone, once the patient has begun breathing halt compressions and monitor, and if they stop breathing again, once again begin doing compressions and continue them until EMS arrives if patient isnt breathing to keep the heart pumping, and EMS can intubate the patient. I had to do this in the middle of the airport when a gentleman suffered a massive cardiac arrest, ultimately it is what saved his life he stopped breathing on me 4 times before EMS arrived.

    Might want to put the "might want to call 911" when explaining that someone is choking, not after blows to the back, abdominal thrusts, repeated. CPR, repeated... 5 mins later might want to call 911 as they have been laying on the floor for quite some time now and we've tried everything.
    Is it prayer then 911? I can never remember.

    I was just choking on acid reflex it was like a little bit of burning liquid throw up and it got in my air way and I started choking but I barley breath it was like short very weezy could barley breath gasps and I almost passed out. Me and my wife were both terrified but I mamaged to start breathing again but I'm just worried cause my wife didn't know what to do so I just want to take any information in case this happens again it wasn't food I was choking on it was the acid so please any tips will be great I'm scared shitless of it happening again

    If you save someones life using only the heimlich maneuver, does he/she tell you that you saved him/her due the wrong protocol? I need to tell this to my wife whom i just saved yesterday using the heimlich maneuver...i forgot to leave the heimlich part away. So the next time she intends to choke off the piece of meat i start to slap her at the backbone meanwhile she dies.

    If food is stuck in trachea which is what causes inability to breathe, "rescue breaths" won't do anything; the air can't get in or out!

    Tell you what the hitting on the back works. I was choking and my friend hit me so fucking hard on my back a chunk of food flew out of my mouth, and Jesus the next day there was a huge bruise on my back but it did work :)

    Back blows work but are also very dangerous to the spine, and other organs, and it’s actually possible that back blows lodge food farther because of how your throat is shape

    In the situation that a person is in need of chest compressions aka cpr. you do not want to stop and give them air that is basically killing them because blood flow is way more important than air flow.

    the AHA now tells people that they don't have to give people breaths anymore unless they have a barrier divider or an oxygen bag or if they want to because it may be unsafe.

    ive saved 3 adult chokers lives,(perhaps my babbies too a few times that got a little scary.
    THANK YOU LORD I have been able to apply aid properly and save lives. Heres my take.... this video is usefull, however, the pace that "shit happens" is frantic and not as sif you can casualy apply aid as the video almost implies. when someone starts choking, you first have to get to them and get yourself in position to actually be able to help them. I have seen tables at restuaraunt wiped clean of everything as a choker lost control and begged for my help.(scary as can be). so you better get to them quick and effiently w/o any delay or emotional stalling delay of any sort.
    You better be ready to hit their back hard and effectively . You gotta get behind them quickly and take control of their body from behind and get the abdominal thrusts started ASAP. DONT sit there asking for responses, just get to work on the choker. Be ready to have the first Heimlick pump NOT work, the 2nd hard pull(not casual) WILL BE THE SAVE THEIR LIFE CHANCE! twice my 2nd tug saved the day. and let me tell you, that time stands still with fear in you as you try to save a choker and fail on the first Hylick pulls. the time from pull 1 to pull 2 has so many hopefull thoughts and pressure on YOU to save a life. It all on YOU now. dont look for other people to help if you are aware of choking Please dont be passive, you will need all your strength and good timing on the abdominal thrusts tugging , especially if the person choking is a big man. you gotta really get your arms around the choker and "dig in " physically when you start Heimlick manuever.., watch out for rib breaking accidentally. get your fist hand pushing the none bones upward just UNDER the ribs. calling 911 better be fast if at all bc a choker is someone that need help ASAP no seconds to spare. GOD Bles and i hope no more choking saves will be needed by my presence as i have successfully saved 3. The 3rd was the hardest as I had to perform 8 thrusts to abdominal, broke a rib of choker, too. if i stopped at five my wife would be dead. i broke her rib and thank God she survived. Cut your steak well and hope this doesnt happen to you. 2 of the 3 were women eating steak big bites fats bc they were hungry. the other wa a large man eating a chicken sandwich.. two of 3 took 2 abdomial pulls aka Hymlick, the 3rd took 8 tugs and was the scariest thing i ever had to do. YOU WILL HAVE TO SAVE THE PEOPLE, not 911! Be strong and focus! this aint an easy thing and you have to create a "YANK" when you perform the abdomial thrusting. remember, you onlu have so many yanks bc each yank loses precious air so make it count when you have to smack a back andbe very quick to Hymlick. God speed.

    There's no theoretical way to create the same amount of pressure on the trachea and press the blockage out by hitting someone on the back as there is by doing a heimlich.

    They forgot to mention to look and do a finger sweep inside his mouth before starting CPR... if there is something stuck in his throat by blowing rescue breaths it wil only make it go deeper.

    Wait, I get Heimlich maneuver, it's named after Heimlich, but 1:01 says now it's called "abdominal thrusts"..they mean somebody's name was "Abdominal"? On a more serious note, it seems like there could be some attachment you could put on the hose of a high powered shop vac that might work, don't know if the suction would do anything but maybe if it had a narrow hose it could actually go down the throat to grab the blockage. That's what really freeks people out when the first start to choke is "do I try to swallow it or do I try to cough it out, I think always cough it out, don't worry about messing up your shirt or getting embarrassed if in public.

    I believe the video's not updated on one thing: while a full CPR is more effective overall, the new 2015 American Heart Association CPR guidelines gives equal importance to "compression-only" CPR for those untrained. Studies found rescue breaths to be ineffective or even harmful when done by an untrained person because
    1. those who never performed CPR before have difficulty in making a complete seal over the mouth and/or breathes too little air in, and
    2. too much air is given and makes the receiver vomit, making the situation worse.
    or so i've heard.
