      YT c€nsoring seems to have shadowbann€d my first comment, so I'm reposting it. 😊 From watching a bunch of YT videos on this subject I'm interested in PRF and Ellacor. Some people say that PRF under the eyes don't work, and others say it works well. But if PRF doesn't work it doesn't need to get dissolved. Which is why I find it an interesting option. Ellacor seems to still be in the testing stage. I am expecting Ellacor to be less operator dependent. Unlike lasers which can ruin your face if the operator isn't properly trained. So therefore Ellacor - IF it works as claimed - should have a much more predictable result. Predictability is such an important factor. 😊

I have heard from colleagues who do PRF and from patients who have done it that it does work and lasts for around 3 months. It’s not something that would give you the same potential issues that filler can, with the post hyaluronic acid recurrent eyelid edema. However, I don’t want to have to inject my under eyes every 3 months and I explained this in that vide, you should check it out 👆 . Also with Ellacor, I do believe any laser or device is operator dependent. Ellacor is kind of new and it said to remove millimeters of sagging skin, but I’m not sure it would give you volume the way that filler or a biostimulators like Sculptra can give volume. Also, I ‘m not sure how predictable it would be because everyone responds and heals differently, and there is some downtime with it.

      In order to be more realistic, you could at least show people before the surgery.Not all these pictures from when they were in there 20s and 30s.And now they're in their 50s and 60s most of the pictures you showed were super old!!!! Obviously they changed in age regardless of the fillers...duhhhhhhh

    I'm intrigued by PRF injection, definitely interested in learning more about it and how it can help mu under eye hollows. I won't get filler there but Sculptra can't be used there, so PRF seems like a good alternative 🤔

      To me it was a disaster. I barely evear swelled with fillers. When i tried PRF i looked like i got into a fight and lost for nearly 20 days. Also, no results after swelling was totally gone

      @@bkpl269 I actually had a very similar experience. I was very bruised and swollen. I looked like a cat. I don’t think everyone has this issue though. I think maybe I got a lot of volume injected.

    i've done both. i think both have a place. i have done bio stimulator but i can't say i've seen a difference. when we pay so much upfront for these injections, we want results immediately. i think if filler is done conservatively than it will look amazing. its a balance!

      Thanks so much for your feedback Angela. I think both have their place also. Curious to know how many times you tried a biostimulator. Immediate results are important and that’s the great thing about filler is it provides immediate results but personally I would rather invest into banking on collagen, because in the long run, I think that’s better. I do think you can do filler and it look natural but at some point I think it’s beneficial to start biostimulators

    I am considering fillers, but I have never had any injectable. No botox or anything else. I am afraid because there are so many injectors nowadays and I don't want to look filled and frozen.

      Better care, if you won't end up disabled, like I did. I was injected by anesthesiologist,and wound up on wcheelchair due to spastic paraparesis, and ventricular arrhythmia. Caused by autoimmune reaction to filler.

    It's bad enough that overfilling gives everyone the same kind of puffy mid face. Now, when they take the fillers out they all have the same 'deflated balloon ' look

      @@behemoththekitty I don’t think the deflation would last that long. If the enzyme dissolves your natural hyaluronic acid it should come back in around 48 hrs.

      My coworker has really bad eye bags and forehead wrinkles and she’s late 20s and spends time in the sun a lot. She recently got Botox and filler but it looks really good on her. But she only gets them done every six months to a year

    Hi Dr Dowe! Love your videos and I’ve learned so much! I’m in a conundrum here where I’m 48 and had 4 vials of Sculptra but I’m less than 3 months out from my 1st procedure. My injector truly believes that the “cheeks are the foundation of the face” and thinks that filler would benefit me. I have really tried to avoid HA fillers and hope that Sculptra would help enough that I won’t need HA fillers. I have a lot of good fat pads and no jowling yet but my main concern is the lines going down either side of my neck under my chin (Cobra neck) starting. She thinks cheek filler will help me. She also said she’s not trying to give me cheeks but just support the ligament to stabilize the mid face. I wish I could feel confident that Sculptra will be my answer to the problem but she thinks it will only help slightly. Thoughts?

      I so appreciate the reply and thanks for the correction on the credentials, I wasn’t very observant. I just think so highly of your skills and knowledge so I always think of you as a doctor! I think I’ll wait 3 whole months (the horror lol) and see how this Sculptra comes along and then consider a consult with you. Thanks again!

    If it were me, I would get hyaluronic acid fillers first (slowly and judiciously, in the right places, skilled injector, natural look and all that)…that way, if I don’t like the look, it will wear off in a year anyway, or I can get it dissolved if I really hate it. It’s also abit cheaper than Sculptra. If I like the look, I can graduate to using Sculptra once the HA fillers wears off. Sculptra is irreversible, you can’t dissolve it if you don’t like it, and it takes 2+ years to wear off, it’s a bigger commitment and requires more patience to see results (and more $$$), so I personally wouldn’t jump into doing it the first time if they have never had fillers done before, and they don’t even know what their face would look like or react with added volume. Another great option other than Sculptra is fat transfer, but it’s a surgical procedure. It’s ultimately cheaper than Sculptra if you have alot of volume loss in your face and it’s 100% natural, the downside is that the recovery is longer (your face will look abnormal for 2 weeks and there will be soreness at the liposuction site) and you must have enough body fat to harvest. I’ve seen people who are too lean get turned away in my old clinic because they don’t have enough fat to do the procedure, and they are usually the ones who need the volume in their faces the most.

I have never had regrets on any of the patients that I have treated with Sculptra. It’s just growing collagen so it does not give the hardened look that filler can.

    Nope girls don't put any type of filler in your face please! My best friend who's like a sister to me a few months ago got filler on her lips (she millennial like myself), she really put a small amount of it and that was it. We met and I saw something was a bit off with her face but I couldn't put a finger on it. We took selfies (I hate pics of myself) and each went back to everyday life. A few days later she sent me the pics she had taken. She really looked even weirder in the pics. My fiance who's in the medical field saw the pics too and he said: oh that's such a pity. She's very good looking prior to the lip filler now she looks weird.
    I talked to her on the phone and she confessed that she got lip fillers and she doesn't remotely like it. She went from gorgeous to an insta fake face model overnight. The woman here says sometimes you cannot notice it. Believe me you can notice even the smallest amount of filler on someone's face. The face looks weird and off even when using smallest amounts. Ladies just embrace the wrinkles. A face with wrinkles that looks natural is always much better than these weird faces we see everyday. Don't ruin your face! Do what the french ladies do, instead of spending money of fillers and whatnot they just use best skin creams out there and even wrinkled look so elegant. They take care of themselves from within.

    Women should consider mini lift than using fillers. Good doctors won’t make people look weird, they just go from tired to rested.

    There’s no reason that anybody needs to do those until they’re 40s. Doing it before then makes you look 40 that’s just the fact if you look at all the people in their 20s who have been getting fillers they all look like they’re in their mid 40s now. Don’t do it.

      @@landy952 Thanks for your comment. It’s possible to do fillers when you are younger than 40 and not look older. Also, keep in mind that some ppl get fillers, not to look younger, but for the purposes of pure enhancement. For instance, a lip filler enhancement, a chin or jawline filler enhancement. They don’t want to wait till there 40 to enhance their look in this way. Another example is under eye filler. That is done not to look younger but less tired as under eye circles can be present from a young age. You just have to find an injector who is conservative in their approach

      @@landy952 Yea there are definitely ppl who do it who you would never know. I agree it’s unfortunate to see movie stars who look unnatural but I guess they were just trying to address the signs of aging or they were doing what they thought would make them look more attractive. The crazy thing is that celebrities don’t always have the best medical providers. Michael Jackson and Matthew Perry are examples. Doctors were prescribing them medications that they should not have been on, or they were prescribed too high doses. Priscilla Presley is another example. She got industrial silicone, I believe injected into her face by a provider. You would think they would have better providers.

      The only other thing I have heard of is silicone which is old, no one really uses that anymore. Hyarluronic acid fillers are the way to go for lips.

    I don't understand how you can tell people to inject things in their face if they desire the most "natural looking result". If someone wants "natural results", they could just...not inject their face. You also don't go into what Bio-stimulators are exactly - they are still compounds you inject in your face, so it's just another fancy word for a type of filler.

    I think fillers should be done by professional who have proper education and who know exactly where and how much filler to put to get optimal results. That is not something that could be done at home or by some friend.

    Because fillers end up costing you money later when you need a face lift to tighten up the skin envelope the fillers stretched out.

      Thanks for your comment. Filler does not stretch the skin out. You can get a facelift after filler. The thing about facelifts are they are great for saggy skin but they do not replace lost volume

      Yeah except people who are fat usually eat a lot of sugar and sugar is sure to cause glycation and age you faster in the long run so be careful with your theory and look it up for yourself.

      @@Glossyvixen I have heard of this theory too but this would be hard to prove because I don’t think there have been any studies to that have looked at the faces of ppl who eat a lot of sugar vs ppl who don’t. Even if it does cause someone to age faster I think the facial fat hides aging.

      I definitely love facial exercises and my face is on the thin side. I’ve built up the muscles in my jaw and cheeks also needing less Botox. You just have to be consistent with it.

      That’s because they’re seeing the long-lasting effects of it. People who got fillers have gotten older and they’re aging terribly because of the filler (can’t say filler is the sole reason they’re aging terribly tho because there are countless other factors involved).

    I had fillers for many years before I stoped doing it due to the reasons that you are mentioning. I tried PRF also and recently fat grafting + nanofat (injections of growth factors of my own fat) and i think this is definitely the best option. It is a surgical procedure and of course, not everyone can affording it but it efficient and long term.

      I agree with fat grafting…I’ve had a combination of micro and nano fat grafting on my entire face (lips and even forehead included) and the amazing results lasted almost 5 years, I looked 25 at 36, and very natural…it was just fantastic. In my country it’s ultimately cheaper than Sculptra because you don’t need to keep coming back for more for atleast a few years, and the results are extremely impressive and almost immediate (after the 2-3 weeks of swelling goes down). I have never really used HA fillers before (tried it only once in my lips when I was 25 and hated the look). I just turned 40 last week, and would love to do another fat grafting session, but I’m leaner than I was 5 years ago so recently my doctor suggested fillers instead, he thinks I look good for my age (so I guess the stem cells that came with the past fat injections made me age slower?), even with the new aging changes. I don’t know if I will do fillers, maybe tiny amounts, maybe…but I’m definitely going to try PRF.

    I did fillers two times in my mid forties-by an RN injector at a plastic surgeon’s office-only in the lines that were forming around my mouth and above my lip. It’s 10 years later and those lines have NEVER come back. My practitioner refused to do filler under the eyes (said skin was too thin) or in the cheeks or jawline-recommended waiting for surgical intervention later. Boy am I glad I listened!!!!

    What about filler at the bone level? Structural volumizing or the Y-Lift. This is very different from regular filler under the skin. I don't see much talk about this procedure.

    As more information is coming out, i think some are questioning benefits versus risks. There was a study recently that fillers MAY cause cancer. Then there's the problems of spreading.
    In addition, there appears to be greater interest in biostimulators cause of suble improvements. Interesting topic, but yes, i believe people are moving away from fillers and seeking more natural injectables, such as Prf or fat transfers. 😊

    People are abandoning overfilling. They are not abandoning fillers. Fillers have a great place and are very effective in certain ways when used appropriately. Obviously, people are learning their lesson with overfilling. But it’s not fillers all together.

    I only had fillers once and they were a disaster- they changed my smile, migrated and when I had them disolved I had a really bad reaction: muscle athrophy, thinning skin and many more. Fillers are unpredictable, no matter what you are saying

If we were told there is even 1% risk to have these issues, I doubt women would go ahead with these injections. They are so unpredictible, is impossible to know how your body will react to them even if you had them before. They have distroyed my life and I cannot find a treatment to stop the progression of my symptoms. It makes me so angry that even if injectors are now aware of these possible risks are still offering this option. How can they sleep at night? I feel like a victim of physical abuse.

    being young means having a skin tight glued to the muscle . with filler distance between muscle and skin increase wich is what we see in old people with skin hanging there is no more tighteness. furthermore filler cannot replace fat loss because fat in hyperdemis is attached to network connective tissue making skin firm and smooth whereas filller in dermis or hypodermis can only distort or detach connective tissue from fat rendering skin non firm destructured and watery or scar hard type (like in scleroderma) depending on using HA or PLLA filler.

    I know I am interested in trying things like thermage, and I don't know if something like chin filler would interfere with that. I currently use filler sparingly and I think the cost honestly prevents me from going nuts. I don't know what would happen to me if I won the lottery..

    Sculptra really saved my face from deflation at 32 years old (last year). I also had some restylane on my cheeks last year and redid it with Radiesse this year and I noticed that there's a very real limit to where my cheeks will look overfilled. Because it seems to me that filler moves in a way that is different from our deep fat pads. The last think I need is to look overfilled. Belotero balance injected with the blanching technique did an amazing job to fix my smile lines though. My nurse is also really talented. But I definitely see the shortcomings of filler and how easy it is to go overboard.

Yes I feel that filler, especially if we solely rely on filler to fill all aspects of volume loss, that it does not move in the same way as our natural tissue. If done sparingly and in certain places of the face perhaps it can move like our natural tissue, but it is a gel that’s sitting in the skin taking up space. This is why I really love biostimulators. I wish we could get more options for biostimulators here in the US.

      Recommendations for skin laxity to the submental area would be based on how much elasticity was lost from weight. I cannot advise without being able to evaluate how much elasticity was lost.

    Fillers hype was a total lie !! Fillers for lifting is a lie .. fillers dissolving overtime is a lie!!

      @@evaelizabeth6186 I would not say it’s an outright lie. We have learned a lot about filler through the years that we may not have known in the past. Filler definitely has its place.and there are a lot of factors that come into play when it comes to fillers dissolving on their own. How many times you have gotten treated plays a big factor, as well as how much, volume wise. If someone has gotten a conservative amount of filler in areas other than their under eyes, there are a lot of instances in which filler may completely dissolve on it’s own. Also in some instances filler can elevate or lift the tissue, even though some say it does not, I have definitely seen instances where the tissue is more raised. Everything is not always black and white but I do think that we have been mislead about certain things and the pharmaceutical industry probably has their part in that.
