Ok so this is what I figured out and thought was logical: His girlfriend was toxic and abusive (which we can see in scenes where she tells him to "put on a happy face" and pushes him into walls) and he might've even been bullied. That would explain the liquid coming out of his eyes and mouth (tears from the eyes, blood from the mouth). It would also be possible that she drugged him in moments that were not shown. That would explain why he sees the fluids coming out of his eyes and mouth as a black liquid instead, his eyes becoming smileys, that other him showing in the beginning and the looks of other peoples faces in different scenes. He could've also tooken the drugs himself in scenes that we didn't get to see or his girlfriend drove him almost insane. In the TW:s in the beginning we can see it say "toxic" and "abuse" hinting to her being both of this and maybe other people being violent towards him. Hope this helped!
