    I want to give this video more than 1 thumbs up. I'm a 66 year old lady with zero calcium (with contrast) who is now metabolically healthy at my mid 1980s weight before children) after changing to low carb, whose LDL went up a lot (not LMHR) and both cardiologist and GP want me on a statin. I've said no thanks. I've watched lots of videos about high LDL and done the various risk calculators and am satisfied that I've made the right decision for me. I've also seen the paper that "Statin therapy is not warranted for a person with high LDL-cholesterol on a low-carbohydrate diet"

      I was doing extended fasting and had my lipid panel done and my doctor's (well nurse's) knee jerk reaction was prescribe water-soluble statins. My HDL was super low, LDL / triglycerides / total chol were all super high (compared to my less than a month previous non-fasting lipid panel) and yet I am no longer "pre-diabetic," whatever that really means. I had to respond about the extend water fasting (~30 days total with a break in the middle) and they asked for a new lipid panel in 3 months. Now I'm doing carnivore, but I'm not sure three months is enough time to actually see a substantial change in some of these numbers. It seems context is lost on technicians and that's all these people are now. Like lawyers, they have canned praxis they rely on but don't know how to use critical thinking to come to more nuanced conclusions. I would expect more from a profession that "bills" far more than they clearly are worth. I for one would be a bad politician as I would clamp down on non-profits and HC companies in this space. There's no reason doctors are worth more than the guys who repair our infrastructure other than the illusion that doctors are revered in our society and the .gov has prevented more entrance / competition in this space.

      @Ashlyn-qg5tt That's interesting. Was it the same doctor? My GP graduated in 2018 and is using the TC/HDL ratio and I told her that I'm using the Trig/HDL ratio. When I said that there are studies showing that statins can cause/worsen diabetes she said (mid 2023) that there are studies showing that it can help diabetes. My thought was (yes, I kept my mouth shut) so which is correct and where are you getting your information from; most likely the pharma rep. I have heard from what appear to be reliable sources that one of the statin trials had a "pre-trial" and anyone who had a bad reaction to the drug were not included in the proper trial. Gwen Olsen was a pharma rep for 15 years and wrote a book Confessions of an Rx Drug Pusher published 2009 (there are some videos with good info) and said that when she went to a teaching session about a new drug she made that comment that one of the side effects was ??? and the response to her concern was oh well we will find a drug to treat that.

      @@pavman42 "I was doing extended fasting and had my lipid panel done" - The fasting period pre blood test for cholesterol is maximum 12-14 hours.

      ​@@pavman42Another poster kind of beat me to it, you don't want to do any testing on an extended fast. Always do the same 12 hours and at the same time of day. Avoid working out or even bed time fun the day before. All these things will elevate certain markers and you will get a sketchy number.
      Cholesterol in particular is crazy, it's almost pointless to measure it. It can be taken multiple times in the same day with vastly different readings depending on what is going on.

    Dr Ovadia is a great mentor teacher and doctor leading by his own example. His book Stay off My Operating Table is a practical and clear educational resource on metabolic syndrome. I appreciate Dr Ovadia and Dr Westman teaching their lessons to all of us online.

    I like that Dr Ovadi makes the point that our heart disease strategy must not be correct if heart disease remains tbe number one cause of death for 30 years. You have to change the strategy to change the outcome.

      It's stupidity to keep doing the same thing and expect to get different results. Going low carb and now very heavily meat based remitted my diabetes, improved my sleep, got rid of my NAFL, off blood pressure drug, waist went from 44" to 36", lost 25% of my starting weight and now weigh what I did in mid 1980s before having children; and that was without even trying, ie no strenuous exercise or calorie counting and I definitely don't go hungry! I'm now 66 and have seen a huge (yes pun intended) difference in the size of people's waistlines over the decades; at 5 foot with a 44" waist I looked like a barrel. Now when I go out I feel quite small compared to the majority of people.

    CAC Scan is underutilized because insurance does not want to pay for it. I coughed up $139 and had it done. Glad I did.

    This discussion sounds like me. I have Int Med & Cardiology physicians telling me i MUST take statins to prevent a heart attack in the next 10 years, but they have not done the tests suggested here to prove that is true. i hope I can convince my cardiologist that the CAC test is warranted to see if statin treatment is really appropriate.

    Fascinating talk. Here in England, our beloved National Health Service hardly ever provides the option of having a CT calcium scan. That is reserved for people who can afford to pay private clinics for the test. Usually, if coronary atherosis is strongly suspected, the NHS cardiologists will refer us for a CT coronary angiogram instead and have the stents ready at the same time!

      CT calcium score scan is not covered by insurance in the US, generally, but only costs $100 or so. It’s widely available.

      I have been eating a ketogenic diet for a year now. I’ve lost 30 pounds, reverses my type two diabetes. I thought I was doing so much better. However, I had an abdominal ultrasound for left side pain. The results showed atherosclerosis in my aorta. I just watched this video so I looked on Dr Ovadia’s website and he has a health coach option that is $495.00 a month. The average person can’t afford that. I feel overwhelmed. I wish these RUclips keto doctors had a listing of MD’s that are pro keto and are willing and able to help the average Joe get help through health insurance. I could ask my doctor to do a CAC or CT angiogram but she isn’t going to know what to do with the info. I’m really discouraged.

    Loving this interview! I checked out the Mayo Clinic statin decision aid mentioned, and noticed that they don't even ask about LDL! I've lost just shy of 116 lbs. with paleo>LCHF>keto and having no trouble keeping it off. My LDL is "high" (125) but my TG:HDL ratio is a spectacular 0.48! I'll turn 67 later this month. A1C is 5.2. Low carb rocks!

    My mother had to have open heart Surgery. 4 bypasses , 2 valves replaced.. She had been seeing all sorts of doctors for years...very much believes Doctors are as close to God as you can get...She Never had high blood pressure ,never had high cholesterol. Not one single Doctor ever detected any issue with her heart. As for why she needed so much fone SUDDENLY they were "unsure" Her Current cardiologist has never mentioned InsulinResistance to her as a concern. They still tell her eat a low fat diet.. And Moderate carbs are okay. Her heart surgeon told her that it wasn't her fault that her heart ended up in such a state. Sigh.....

      Won't do a Keto diet. Her doctors tell her she doesn't need to change anything because she has stable blood sugar eating the way she does. They told her that her heart disease had nothing to do with the way she eats or anything she did. She has been a type 2 diabetic for 40 years!! But never put on insulin. Just Metformin because her A1c stays at 5.9. She also believes their advice that as long as she keeps to calorie restriction and low fat that's the best way for her. Never mind that all that lack of good nutrition has caused a big handful of other problems like chronic neuropathy, neck fusions , spinal fusions and both knees replaced., Her vision is also really bad, nearly blind. She absolutely believes doctors when they tell her this condition
      It is genetic for the most part And progressive no matter what she does. .
      I have talked to her about insulin resistance, Especially After I found out I have insulin resistance. I have explained to her that all that inflammation and Arthritis that she complains all the time Is connected to insulin resistance, And heart failure. She knows used to have Inflammation and Stiffness In my joints which I don't have anymore because of me going keto.... And that it's helping reduce my insulin resistance and risk of heart disease.
      She just trusts her doctors more than anything else.
      She believes she's only supposed to have 4 ounces of protein a day maximum.. Because that's also what they taught her.
      Seriously.Hope someday somebody sues the hell out of the American Diabetes Association for all the bad advice They give people when referred to them by their doctors.

      @@warrenrae32 She had supposedly one of the best heart surgeons ever. Dr Luis Castro. At Sequoia Hospital... He and his yeam did a phenomenal job
      Saving her life. None of his team could believe she actually walked in to that hospital the day she had surgery. She's a tough cookie she's the only person I know that managed recovery from open heart surgery just using tylenol.. She's allergic to all other pain meds.

      Sadly, my father is mid 70s, healthy his entire life and avoided doctors, but after turning 70 started going down the pharma route. Now taking the jabs, on all types of pills, statins included, and his health is deteriorating rapidly. Nothing I say will ever come between him and his doctor, it's pointless to even try. Maybe that will change, maybe not.
      He is however skeptically watching my health go through the roof with a ketogenic diet. Lead by example!

    I am 70 and my CAC score is 0 and my doctor regularly tries to get me on statins because my cholesterol is a “little elevated”. And I keep telling him no.

    I wish there was more focus on those with CVD. This interview had more than others I have seen, but what is the recommendation and expectation? Is Keto/Carnivore the best diet recommendation? What is the expected progression of CAC score on Carnivore? Is it still 15-20%? Is there any data? Are there strategies to reverse progression? How often does this occur? Do we understand the mechanism of calcification? etc. etc.

      Check out Malcolm Kendrick; Ford Brewer, Agatston; Bart Kay; Peter Attia, Dayspring, linus Pauling, Thomas levy to hear the multiple povs on the reasons behind atherosclerosis. My feeling is there is quite a debate and the studies so far are conclusive and not conclusive depending on the expert. So really it's up to you to figure out which lines of thought make most sense to you. This is clearly not an open and shut case.

      Also the book The Unholy Trinity covers a lot of what you're asking about in terms of reversal or slowing progression. I think only Linus Pauling's Vitamin C work is not really mentioned in that book. So it's worth taking a look at what linus pauling theorized when the whole LDL diet heart hypothesis took hold.

    It would be good if you could cover hereditary risk factors. I have had a few close family members who have either had heart attacks or had to have heart work (and in one very close relative, both); however, I'm still in early stages of potential impact but have altered my diet for years to try to lower my risk. Sadly, my CaC score wasn't 0, but it was well below 100. I'm trying to figure out if this is higher risk or if it's not a huge deal based on what my previous lifestyle was until I went a different direction from "the ancestral pack." Ultimately, are heredity risk factors more or less important than dietary adjustments to lower IR, A1C and ultimately carbohydrate dependency?

    I do apologize concerning my criticism of Dr West and other Holistic Influencers approach concerning ketogenesis. I think it's important for people to understand that ketogenesis is a baseline metabolism that is natural and not a made up alternative fad. Depending on a person's physiology and their mother's dietary habits while pregnant they may or may not be born with this baseline metabolism. But given that natural fasting resets the body's metabolism back to ketogenic speaks volumes.

    The church of, El DL. I just had an advanced cholesterol panel. 3 year carnivore, 1243 cac score. I passed my functional test the doctors said 95% of max/no heart disease. Total 277, trig 38, hdl 76, ldl 193. Ldl partical nmr 1554 mo1/L
    Small ldl

      My hdl to triglycerides ratio is small.
      At this point, who knows what will happen to me.

      ​@Ashlyn-qg5tt I don't think CAC score defines heart disease. Correct me if I'm wrong on that. The problem with a CAC test is that it measures the calcium but does not indicate where the calcium is. Some people have a really high score but when they're cathed, all the calcium is on the outside and their arteries are fine.

    Great episode, but mammograms for early detection of cancer may not be the best analogy due to questionable efficacy of mammograms. The point makes sense, but perhaps choose a different example.

    Had calcium score feb 2023 600 did all meat diet just got test again last month apr 2024 sore was 699 and yes, took all the vitamins vitamin K vitamin B‘s D’s antioxidants q10 now going low carb low but good fats higher protein and see what happens next year

    I have 4 stents after I had gone through an Echo, EKG (abnormal) & a scan. Got the scan because of the EKG. After no feedback, I called & was told my scan "was fine." My heart attack was 6 weeks later. So I have to wonder whether the scan was legitimate or something the cardiologist ordered. Minimal faith in cardiologists in general & I always ask about how my 'clogs' occurred but have gotten pedestrian explanations.

I asked the cardiologist why they didn't see the blockages & was told that because I had 4 blockages the scan wouldn't pick up on the problem. Is that BS or what?!? Whatever they did, it obviously wasn't the scans these doctors were talking about.

    Let's do experiment : take 2 groups young healthy people. Before experiment measure theirs lipids, do coronary arterys Calcium score. Do even Ct angiogram, coronarography. Put 1 group for 3 years on high animal fat red meat diet and other grup on vegan diet . Than... repeat Calcium score & Coronarography... Lipids panel..Why not?

      Yes. Stop taking them. Take k2 as it directs the calcium out of soft tissue (like arteries) and puts it in your bones. If you're low in vit d, take a vit d supplement with k2 added.

      From what I know, I wouldn't touch the stuff. If trying to correct osteoporosis, dump the plants. They are notorious for sucking calcium and other minerals out of the bones while blocking absorption from diet.

    One thing that is not mentioned is how Dr. Ovadia defines a ketogenic diet. Does he use a scientific definition such as a diet that induces the production of at least .5mmol/L of ketones or does he use the term Keto like Dr. Westman to describe an imagined concept without any measurement?

      25 grams of carbs a day or less, it's that simple.
      Even a carnivore meal will kick the body out of ketosis for a short period of time.

I'm sure his diet is fine, but Keto is a word that relates to ketosis and he could care less about ketosis. He says this many times and in published papers. All he has to do is use the term low-carb if he wants to be accurate. My comment has nothing to do with the effectiveness of his diet, only what he calls it.

    I can understand to cut carbs...but ...Replace them with red fat meat, cholesterol, lard, butter ,eggs, bacon ?😮

    Do you really need a clinical study to “prove” basic physiology? If there is no such thing as an essential carbohydrate why would there ever need to be a clinical study proving that a low/no carb diet is safe?

    You were prediabetic? That's defined by blood sugar. Maybe you are hyperinsulinmia that's defined by blood insulin, measurable by c peptide.
