I think the first Continental Hotels were originally Assassin Bureaus (small bases). - The Bureaus were transformed into High-Classed Inns. High-classed Inns transformed into expensive hotels (Continental Hotels).

Given the truly ancient heritages of Italy, Japan, France, Britain, Greece, Morocco, etc., it's quite possible that certain hostels/hotels dating as far back as the European and Japanese Renaissance might have been prototypes of The Continental Hotel chain, meaning the Italian, Greek and Japanese Continentals might be updates of older established hostels/hotels existing as long as 450 to 500 years ago, which were taken over by the various powerful crime syndicates, renamed and remodeled to elevate their design and quality standards of accommodations, entertainments, amenities and services meant for all those wealthy and/or powerful enough to afford them - again, depending on an assassin's or civilian's financial and socio-political standing relative to whichever ruling plutocratic/kleptocratic oligarchy ruled the regional territory served by each Continental (The Russka Roma, the Camorro (?) Combined with the the fact that we only see local cops ignoring criminal activities, I think it’s safe to presume the Continental and High Table are wary of Federal law enforcement. In the real world, a similar thing happened in gangland Chicago with Capone; he was virtually untouchable on his home turf, but the Fed eventually brought him down.
In terms of how many one could speculate from the size of a country and how many major cities it has. So for the US there is New York but maybe also Hollywood, Washington DC, and perhaps one in a more centralized state. Paris and Berlin are also good examples, perhaps one in Cairo and Beijing respectively?
