The Netherlands have a very dense highway network without traffic lights, crossings, or slow traffic. One of the safest in the world.
But there are other networks like trains, also with very limited crossings, so separated as much as possible.
Cities are relatively dense, and highways go around them, and you pick your exit as close to where you're going.
Separation of means of transport is key.

I love it. Keeping these things separate prevent interference. I've seen a terrible example of this in California right near the Mexican border - a bunch of vehicular traffic is jammed straight up with a train & all mashed up together with pedestrian traffic. Everyone interferes with each other which creates a disaster.
