Wow,this is an amazing video! I am not a nurse,I am a lab scientist and blood banker,so my view of this subject is very different. I watched this video to understand the floor's POV so I could provide better service and have a common ground explanation. I will share this with my lab so the lab and floor can all speak the same language, lol. Thanks again!
references of the subject is only available in clinical trainings / Ng updates should one will consider a career in icu whilst the same is not taught in Ng School.
I so wish that you would consider venturing outside critical care and covering general nursing school topics. I have gotten so much out of your lessons. I am understanding topics that were incomprehensible during my classes. Watching these videos, I am connecting dots. Best nursing channel on RUclips-by far!!
Great video, thanks for education everyone on these important topics for patient care. One thing I noticed in this video minute 8:14 describes that 1 unit of platelets(6pack) volume is only 50 mLs... when to my knowledge a 6 pack should have around 200-300 mLs. Thanks for any feedback.
I really love watching all your videos, and it definitely puts a lot of things in perspective for me especially since I am new in a HFICU. What would you think about doing some VAD/Impella type videos? I love what you are doing! Please keep it up.
You are great.... Thanku for giving uss soo much study material for freee.. m a nursing student from India and u help me a lot...... uptill now as much vedios I have watched are superrrr. Amazed thanku ❤️❤️❤️❤️ plzzz make a vedio on intubation and extubation done in ICU I'll be really great full to u
Awesome! Glad to hear the videos have been helpful for. I really appreciate the great comment. I do have intubation stuff covered in the Airway Management playlist: ruclips.net/p/PL2oVjKTYocdOKRCJ0QmkHF_-yOse8JrgB Nothing on extubation yet, but is on the todo list.
Man I've been in healthcare for a long time and I come to refresh on certain things or if I'm going to have student's or something. These videos make me realize how much I forget and just how much there is to know. Kinda freaks me out. So you newbies don't sweat it.
I always learn a new tidbit from your videos! For platelets, since we do not need a pump for that transfusion, are we still required to use blood tubing or just a regular primary tubing? Also, I never knew that some hospitals required a blood transfusion consent for albumin! 🤯
Awesome! I always try to think of some things to include that may be helpful so really glad to hear this. For platelets, though we still do need to blood set for the filter. And yeah, it is weird. Only one hospital I worked out did this, large system. So weird. Literally every other place has been just like any other fluid/med.
@@ICUAdvantage Our hospital never needed consent for albumin, but it wasnt until I spoke with some travel nurses that I found out that some hospitals require them.
Honestly I've heard both pump is ok and not ok. Definitely no for rapid transfuser. I've always just defaulted to free hanging as there shouldn't be an issue.
I know it was mentioned that there are no antibodies in Prothrombin Complex Concentrate (PCC), but I just want to ask, does this mean that NO BLOOD TYPE/CROSSMATCH is needed for administering PCC? Thank you!
Hi Eddie! I was wondering if you can do a breakdown of the transfusion rates these blood products are normally set at or at what rate you normally start
So the rates really vary situationally and can go all the way to as fast as you possibly can get it in. Let me stew on this and see if theres something I could do.
Also been hearing that TXA is extremely useful for stopping bleeding in trauma, would cyro or TXA be more effective? If one is better than the other why?
Yeah, TXA can certainly have a benefit in trauma. As for which is more beneficial, it really depends what is happening. They both have their advantages, and they certainly aren't a one or the other choice. I do have a good video on TXA that I'll link here for you: ruclips.net/video/1GQLjWY6u68/видео.html
Hope in almost every topic you'll include some common problems encountered and troubleshooting techniques based on experience and other stuff connected with that.please.think it would be most helpful.keep up the good work on this channel.thanks! Please include the duration of transfusion of each blood product and the corresponding administration set.
So I been wondering in the context of trauma what's the point of giving plasma, PRBC's ect when you can give fresh whole blood with a combination of all of these? Doesn't whole blood just contain everything your body normally needs to clot and keep your body alive?
Really a great question. In fact, the military will often use whole blood, as well as some places do in trauma. In cases of massive bleeding, we are essentially giving whole blood in parts. I think it's just used in far less situations and has a greater risk of being wasted, where as the different parts are used all the time. That's my best guess.
Plz I just want 2 know when 2 use albumin 5 percent,, and 25 percent,, which is volume xpander When we add albumin 25 percent to saline or glucose, If this happened,, does it mean that it's converted to 5 percent
cool Educational Videos , as a float Nurse , do encounter a large variety of patients , just had a patient ( 22 year old male ) that lost his left arm in a traffic accident & MD orders administer four units of PRBC . Work in the ICU & MST & much enjoy lots of learning , Ronald J.RN .
Thank you so much Ronald. Sounds like you get to see quite a bit which can be such great learning opportunities! Glad you are finding these videos helpful for you!
It technically is in massive hemorrhage, but for most people we a transfusing based on a deficiency in just hgb and not a need to replace platelets or plasma necessarily. Therefore we can do that just with the PRBCs and have the other products for people who need those specifically.
I'm sorry you feel this way. I try to be fair and what I manually place is much less than RUclips automatically places. The ads are what keep this channel going. Believe it or not, these videos take a lot of time and effort, and I still work full time.
Checked out the quiz on your website, a good idea to reinforce learning. But I was disappointed that name and email address are being harvested prior to being able to see quiz results. This makes it seem like your motivations are a lot less about teaching and a lot more about something else. I hope you transition to a less coercive and more opt-in practice in the future.
I’m sorry you feel this way. I do think it’s very reasonable to ask for this info. I put a lot of time and energy into this channel and the hope is that as I build out more in the future that I’d like to communicate with those who have interacted with my content. I could very well put everything up behind a paywall like others, but that’s not my intention. I also do giveaways for those who complete the quizzes so I would be unable to contact someone. As for the placement, the software that comes with the platform I use doesn’t allow me to change how it asks for someone’s email, so I can’t have it ask upfront as I would have preferred.

Great Video. You can make it better by using SI units to capture audiences outside the USA.
Thanks for the suggestion. I'll keep that in mind for the future, but often times I don't even know when different units are even used...
Very well explained

I used PCC once for eliquis reversal in a patient with a massive GIB. I’m surprised it’s not used more often. Is that just because it’s expensive?

It's quite expensive, and it wasn't something I've seen used often. But it has its benefits.
@@ICUAdvantage I see, thank you

I think you should talk about blood product/transfusion reaction, also you should talk about EVDmanagement and how to wean EVD
Thanks for the suggestions! I did cover the products/reactions in the video that came after this one.
Thanks
But one unit of platelets will rise the count about 5000
I was referring to a 6 pack, which is typically what is given. The numbers vary based on source, but increase is close to 50,000 for the whole 6 pack.
Great video! Love the channel.
Thank you Marlissa and really glad you liked it!
In vet med we use EDTA as the additive for our pRBCs. Is there a reason why it's not used in human medicine?
I’ve seen platelets free hang but we can run it in a pump.
I also have seen both. We are not supposed to use platelets in the rapid transfuser, but I thought iv pumps were ok.
Honestly I've heard both pump is ok and not ok. Definitely no for rapid transfuser. I've always just defaulted to free hanging as there shouldn't be an issue.

About the platelets, shouldn't they also be given in like 20 mins, so that they "activate" properly?
I know it was mentioned that there are no antibodies in Prothrombin Complex Concentrate (PCC), but I just want to ask, does this mean that NO BLOOD TYPE/CROSSMATCH is needed for administering PCC? Thank you!

I recently cared for a pt who was hypotensive & needed 2u prbc & 1u plts. I gave the procucts over 2.5 -3hrs. Could I have given them faster?
Thanks. Excelent work.

55% not 56% 6:21
