It really depends on your level of experience with the rubber dam. I like placing the dam before hand, this allows you to “stop” your preparation before impinging into the dam and use teflon to separate it to continue with the bur. Now, if I know the preparation will be very deep prior to removal of the old restoration, then I remove it first and deal with the isolation later. Hope this helps and thanks for watching
How many teeth do you anasthesize in the multiple isolation technique? Is the clamped tooth and the tooth to be restored enough? And are teeth involved anasthesized both buccopalately?
The clamped teeth are usually numbed, but not all the time. I would say 80% of times. And yes, the tooth to be restored if we are cutting into dentin. If it is an esthetic restoration, no anesthesia is used for those teeth.
One of my all time favorite clamps - modified 12a and 13a. Use a 557 Carbide bur, cut the wings off of the clamps, make them round (think W3 style) and then smooth edges with a Brownie point. Just like that the 12a/13a become incredibly useful

My question is, how do we maintain good suction within the oral cavity? Does the assistant need to intermittently raise the edge and suction saliva? Or leave the aspirator in place at the back of the mouth? Please guide me on this matter. Thanks!
Both options you mention are good. I personally use the intermittent one since I find the mouth over-dries when it is constant. The rubber dam allows the patient to help us with swallowing as well. Glad you liked it, please share.

When I try to use the floss ligature technique to invert the rubber dam, sometimes the floss doesn't go apically enough and sometimes the floss slips beneath the rubber dam (instead of pressing it apically). What can I do to correct this? 2. In some anterior teeth with short clinical crowns, sometimes there is no cervical undercut. When I tighten my knot, the loop slips incisally instead of cervically. What should I do in these situations? Thanks a lot for your sharing.
1. Try using a smaller perforation size. I only recommend the one next to the largest size for molars and the one next to the smallest size for premolars in general. Another option is to use a retraction clamp like the B4 or a modified 212. You can watch this on our “advanced rubber dam” webinar. 2. Again, try the retraction clamps. Completely understand the feeling, this is why we created this content, so that dentist improve their techniques and protect their patients. There are many “small” instruments we use that can fall easily to the back of the throat with bad consequences. Thanks for your comment.
No. 18 or 28 are very hard to isolate, but “if possible” you can try using a 13A or a 12A, really good clamps. If 8 is absent you will have to use 7 for your clamp and choose a retainer-less type of band. Hope this helps.

You really don’t need too much. I haven’t had a patient ever complaint. Shaving cream (not gel) is water soluble so very easy to rinse.
