I have an upper first molor tooth that was filled about 5 years ago. It has a slight sensative feeling if I tap on it from the bottom. Not much of this sensitivity if I tap from the side. No sensativity when biting down or hot, or cold on this tooth. There is some periodontitis around this tooth I believe. What would you say this could be?

Sir wat if their is mild discoloration after long standing trauma with the sensibility test giving positive response both cold test as well as EPT sometimes and patient being asymptomatic.
Should we jump to decision RCT based on discoloration or keep on observation?
Yes. Its better to do rct in that case. There are two types of discolouration.. one calcific metamorphosis and another necrosis.
Both cases need rct since its easier to arrest the colour change now than later
If the tooth is yellow then it may lead to calcification and future endodontic treatment may not be possible.

Not. Not yet.
Sir what about the Test Cavity Preparation to know the vitality?
U take small but and start making a small hole, if the ot feels pain then it means the tooth is vital. This has to be followed by restoration of the cavity u made. If there is no pain and u reach pulp while drilling then it means tooth is dead. So this is meant as test cavity. Very simple make cavity to test if the tooth is vital

Currently these two modalities are for available for other body parts. But for dental pulp these are still in experimental stage and nor commercially available

Sir why cold tests are consideted superior to heat tests.
Because they have good sensitivity and specificity. Read about these two terminology to understand what they mean

Sir then why in our ug days we where told to use EPT or thermal test to check the vitality.. As there is lots of bias and they say cold test Is more appropriate than them and as u said these are neural sensibility ans not pulp vascularity test . ... But instead of choosing one isn't it both has its own advantage tell neural as well as vascularity
So sir then in clinics we need to combine the both test and which tets is preferred in which situation can u tell about this sir???? 🥺🥺
Nerve may be numb and tooth may be still vital. But the true vitality test are still experimental. Also, since we have lot of data on how to interpret nerve response we can still use it properly if you know how to avoid false positive and false negative response.

Overall the cold test is better than hot and electrical test.

Sir very good explanation, i appreciate that I have 1 question plse make clear .... What does EPT test?
a. Presence of inflammation in PDL or not
b. Spread of inflammation to periodontium from PDL or not
c. Responsiveness (does not test vitality (i.e. necrotic, reversible pulpitis, etc.)
d.Pulp vitality..... for this question I think it’s 3 right
Heat and electric pulp test only checks the nerve supply
