How do doctors not know this? Why aren't they taught how this can compromise a person's health? This has been going on as almost a decade and they say chiro (who just wants to manipulate bones) or PT that just wants you to exercise. It's very hard to move and the body compensates by not moving to keep pain to a minimum yet they want you to move. I was very active and when I started having issues and seeking answers from them they allowed it to continue to today. I’ve been having serious neck pain along my vertebrae right up to the back of my skull. I have a lumbar fusion with Harrington rods. I developed a slight lumbar scoliosis and a lateral pelvic tilt 25 years after my surgery. Physical therapy has helped because I used to be hunched over to the left. I just feel like I’m not moving forward and it’s so frustrating.

You’re explaining all the neuromyofascial connections beautifully whereas Chinese medicine aligns so well with more of a psycho emotional bent- that would be an interesting comparison to look at how we view the causation of these patterns.
Thanks Neal. I have been like this for many years. Mine is so extreme that my swallowing feels crooked. Trap, scm and scalene pain on the right. Neck tilted to the right. Right shoulder more rounded to the point the trap has lost its proper function. I don’t even want start my vision issues. I do daily breathing to expand right rib cage. God bless you.

I have had an imbalance that was almost exactly like this however I also have congenital mirror movement disorder which doesn't help so I don't really know how I would even be able to approach this issue.
I have everything you described in this video but on the opposite side. It happened after a bike accident, that left my right side paralyzed. I couldn't use my right diaphragm to breathe until I managed to unpinch some nerves. How do I retrain my body to engage in the right and get balanced again? Will it ever fully go away? It's been a years since the nerves unpinched and I am making good progress in recovery, but I feel so hopeless some days too, especially when I sleep, it seems to reset all over some days more than others. Feels like am locked in a wrestle match with my body to get her back to normal.
My left side is more dominant than my right side. ive even taken measurements and each body part on my left side is about 2 cm bigger. I even have four abs on my left side and only three on my right .
hey, i get gas popping sound from my stomach when i flex my left side and breath at same time, can it put pressure on my organs when i squeez my left abs and rib too hard? other wise thank you for your support, keep up the good work!
Neal...is the "gas popping" sound a result of achieving a domed left diaphragm, which in turn creates pressure on the fundus of the stomach which facilitates gastric emptying?
What has temporarily helped with neck pain is sleeping with a smaller/lower pillow. I would wake up in so much neck pain and headaches. I stopped using two pillows and just used a smaller one and bingo.
This is exactly me;) I just want to add that it can lead to costochondritis and obviously spine being twisted and scoliosis. Neal saving so many lives from pain. Thank you. What has been helping me immensely is inverting on an inversion table and doing a deep diaphragmatic breathing as Neal explains. This is just 'a lazy version':). And yes, intercostal muscles can be painful as hell. There are so many nerves around that area. This is one of my fav videos as it helped me understand my problem so much more. Many, many thanks.
@@NealHallinan Yes Neal. I know that your diaphragm works differently in that position. But do you think it is a problem in regard to air flow distribution?
SIR, I HAVE ALL OF THIS AND NEITHER OF MY DIAPHRAGMS ARE WORKING. IM STUCK IN CLAVICULAR BREATHING WHICH HAS PULLED UP ON MY RIBS SO MUCH THAT IT BROKE AWAY FROM MY STERNUM. MY RIBCAGE CANNOT MOVE DOWN AND IS SO FAR UP IT IS PINCHING NERVES IN MY ARM AND SHOULDER AND MY RUGHT SIDE HAS ATROPHIED TO ALMOST NOTHING. MY NECK, HEAD, AND FACE ARE EXCRUCIATING EVERY MORNING AND I STOP BREATHING A FEW TIMES A NIGHT. I HAD THIS ISSUE BUT CONTINUED TO TRAIN AND FIGHT MMA AND NOW IM SO TWISTED UP MY BODY IS LITERALLY KILLING ME. PLEASE HELP!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!
Albany Oregon. I was 180 pounds of pure muscle, in 2 years I’m at 140 and my right side has massive atrophy and I can’t lift my right arm. I think from breathing through my neck only all the muscles lifted my ribcage into my collar bones. I don’t know, but I’m in serious trouble my friend. I’ve seen countless doctors and chiros but nothing has helped.
Neal, I’m on Facebook and messenger as Rod Jaques, I can send you my cell number somehow as well. Even if you can’t treat me I would love someone to be able to talk to.
Hey Neal, thanks for the great video once again. If you're taking video suggestions, could you make a video about how you approach different ISAs, and the type of exhales you use, if it even affects your approach, or just reply to this if you can. Thanks a lot, all the best.
Honestly, ISAs don't figure into my approach. I do know where it comes from, but it just doesn't fit into the context from which I view things. I treat the underlying patterns, patterns that are produced by the brain utilizing sensory input. I don't treat the physical appearance. The visual presentation is often misleading. Of course, my approach is based on my own personal experiences, something I obviously can't separate myself from. Other people may (and clearly do) have different opinions.
Unfortunately my lower R ribs are malformed due to a fall when I was a child. This causes issues with my R bucket handle motion. Also due to being overweight then allot my arm was checked and not my ribs. I can release the area but the relief is temporary due to faulty loading around the area. If you wish to see pictures or a video or to use me as a case study in a video feel free.
Is it possible to be all tight on the right side but only have left sided body pain? From basically being spasmed in lengthening position (left side is where i only get pain but i KNOW my right side is tight and overactive although theres little Pain)
Hi Neal, thank you so much for uploading these great videos. Now I know what is the name of my pattern 🙂 i have had all these issues you mentioned in this video and had numerous physical therapy and chiro sessions which didn’t help at all. I started developing body awareness and contracting muslces until 2 months ago I found a good amount of relief. Without knowing your techniques, I started engaing my left abs and that was a game changer, the right side turned off about 80% and the left si joint got much more stable. Now I am 80% corrected and I feel that i’m still not 100%. My biggest fear is that I am over doing these exercises and I don’t want to get stuck in the opposite pattern. Do you recommend to keep doing these exercises until I feel 100%? Or should I stop here and let my body correct the remaining 20% of the musclular imbalances thru daily activities (ex: walking, sitting, doing daily chores, etc.? Thank you so much for sharing this.
I encourage everyone to experiment. You learn from "success" and "failure". You won't get stuck in the opposite pattern, the right side will always be dominant. If you are in the US, you may want to find a PRI provider to help you out.
my left hip is higher and right shoulder and dropped and I have more muscle on my left shoulder than my right any suggestions and my shoulders are uneven I first thought it was an imbalance and I would go to the gym and focus on the right shoulder but that wouldn't help in fixing the imbalance do you have any idea what is wrong structure and what is causing this and what I cant do to fix this?
You could say that, yes. The acquisition of a left ZOA implies that the dominant right side has been inhibited. But if the right side doesn't inhibit, you can't get a left ZOA.
Hi Neal, question for you. I have all the problems you’re describing, but on the left side. I’ve seen a PRC therapist for quite some time now, including many FRS therapists but am still having difficulty with a L shoulder impingement, L SCM discomfort, L lat tightness. Any suggestions?? Also, same as your client in this video.. I do shift to the right still. When I do PRI expansion through right chest wall, it’s still significantly tight. Thank you so much!
I have the same problem. I have all of the problrms named in the video but on the LEFT side. Tight left lat, legt tral that is killing me, tight and painful left neck, forward and tight left shoulder, and left rib flare. It makes me so confused.
Hello Jacob. What you are describing is normal, but the nature of the issue is different. The right side is compressed, causing direct compression. The left side is the opposite. It is de-compressed, causing compensatory tension created by the nervous system because no compression is being sensed on the left side. Basically, in the absence of compression, your brain will create tension in order to "keep things together" on that side. The entire left side of the body is stuck in a state of external rotation, which is decompression and lack of stability. The tension/pain you feel on the left (which is compensatory) is due to the fact that you are still being pulled to the right by the right sided patterning. Your left side is dying for compression. That's the way I look at it.
yep me too, all my left side is jacked up, but i still feel like i do stand mostly on my right foot, its my left neck, shoulder, pec, lat, and rotator cuffs that are in pain and left side teeth that clench.
Hi Neal. Can all of the dysfunction your mentioning be on the left side? All my pain is left sided, neck, pecs scapular. I do have some tightness /weakness on my right side but also on my left. When I do your right glute max exercise, I do feel my right side open up, feels great, my left posterior ribs I can’t feel any air going into them no matter which of your exercises I do. I try to set up perfectly when doing your right glute max exercise, though when I’am done, the area next to my spine hurts/burns. I have pinching in the front of my left hip and I still really cannot feel the right glute. I always seem to do chest breathing on the left but not the right. I will continue forging ahead. At least my diaphragmatic breathing is getting somewhat better.
I have arterial tos and am a baseball player. Pri has enabled me to throw a baseball again after two shoulder surgeries. However, I want to continue developing strength and power through weight training. Bench, specifically, is correlated with higher throwing and hitting velocities. I am wondering if it's possible to Bench and still reap the benefits of pri?
No, it is not. You still need the techniques to help your brain "re-learn" how to not use the right subclavius for accessory breathing and stabilization.
Any tips to get air into right chestwall better than right lower back/abs. Even with 9090 with right arm overhead and ribs down air is still going into my right abs/lower back better than right chest
Neal, are the right thoracic erector spinae muscles implicated in the LeftAIC-RightBC pattern? Like the longissimus and spinalis muscles? I notice that these muscles get tight for me and often pull me out of position.
Hi Neal, do you have any tips or a noteworthy comment about dealing with a flat thoracic spine? Most of my other pains have gone away with my PRI program, but I still have pain in my thoracic spine (T8-T4 roughly). I'm sure proper airflow to the right anterior chest wall should do it, but I've been having trouble for a while and I'm perplexed. Anyways, thanks for this video and all of your other ones. I started studying for my CSCS two weeks ago and I'm hoping to get my PRT certification in January 2021, and it all started with your videos!
Hello Tyler. Are you working with a PRI therapist (can't recall?). My guess is that you are still overusing your neck for breathing and/or stabilization.
@@NealHallinan I think you're right. I'm not positive, but I think I have a habitual malocclusion. Maintaining proper tongue posture seems to allow my neck and ribs to move in and out of neutrality

I suffered a repetitive injury strain in my right QL and now I'm feeling pain and tightness in my right ribs/intercostals, glutes, mid back, SI. Would these types of exercises help? My right side is always tense and I feel like I've tried everything but it dosnt want to let go. Also I'm a side sleeper, is there a certain side I should sleep on to help ease up the tightness? Thanks!
I had sent you an email, didn’t get response. Didn’t hear back. Hv polio with leg length discrepancy and my right side is weakened because it’s affected my hip and my right leg. I really need exercises to correct these issues. Can you help?
Hello Neal, I have recently started watching your videos and I can relate it to my problem. I have one question whole life I have used my left leg to stand in place of left leg that means my brain is sensing left side more. Thanks your videos are really awesome.
Hello Rishab. You may be standing on your left leg, but your pelvis/leg are most likely being stabilized with the wrong muscles. You are probably using hip flexors and lower back muscles to stabilize rather than left hamstring, left adductor, glute etc...
@@NealHallinan yes my hip flexors are in spasms continuosly. And I am getting nerve pain all over my body as I can feel my pelvis is tucked both sides and also I am not breathing from my diaphragm rib cage is main issue. My neck muscles are doing work of my breathing. I have started balloon excercise to release my rib cage and my reflux symptoms are improving. Thanks
You might find my story interesting. I have a pelvic tilt of 14.6mm(half inch). I’ve been a mess with low back disc issues since high school and then my neck in the past 12yrs. I had cervical fusion on my C5 C6. My L5 and sciatic are still a mess, my trap muscles are incredibly tight and I have headaches all the time. I can feel muscle pain in my head and temples, my eyes. Tell me, was it a huge mistake to have cervical fusion done with having a pelvic tilt? Honestly I’m miserable, I have constant trigger points all over my body in the craziest places. Across my chest, down my triceps, forearms, abs, all over my legs, calves, my ribs and all those little muscles, my lats. Is it making it worse that I had a fusion done and my body can’t compensate? Or is it something else? I just found out after doing a barium swallow that the hardware from my fusion is pushing on my esophagus. So I was hoping that would be the culprit. Because if it’s the fusion fighting against my pelvis and causing pain all over and these trigger points that’s bad news bc fusion is forever and my hips are very messed up. What do you think? I tried to tell my surgeon about my hips and how I’d been treated by my chiropractor for it for years and he didn’t even respond or acknowledge it. That was before I had my fusion so if I’m screwed now and there’s nothing I can do for it im just stressing. I feel like so exhausted I can’t enjoy anything, it’s been a mess for most my life. I’d appreciate your suggestions. Thank you
Great Video, though there is something I don't understand. You said in another video that 99,9% will have the Left AIC pattern, so the right (thoracic) side needs inhibiting. I have breathing and hip issues on my left side for 10+ years maybe more. However, I dont have any neck or musculature issues on my right side at ALL. My teeth (sometimes), neck, thoracic, subclav, abdominal, quadratus, oblique and hip all hurt on the left side, not the right. So am I a Right AIC pattern? On the other hand my left leg is a little bit too short according to screenings. And my hip pain makes symptoms like it would be Left AIC pattern. But i cant stress enough how much pain and tension i have in all of my left sided muscles of the back, shoulder and neck.
Hi Neal, as always great content! I managed to open up my right side by deeply massaging my sinus areas on the face, predominantly on the right side. It was a feeling like all the way down my right part was finally melting, grounding was at place. Yet, when sinuses are stuffed up again (hell knows why), the cranial tension returns, which again holds up the right neck, shoulder, rib cage, hip, knee and ankle fascial tissues. It seems I need to control or prevent the mucus-forming pattern on my right face. Working on the left side does not drain it properly.
@@andreygladikov8860 A couple of options that have helped me if you have not tried are N-acetyl-cysteine and mouth taping at night. Also using horseradish or wasabi daily seems to keep things open
Andrey Gladikov I have histamine intolerance issues. Usually anti-histamines help me. There's one nasal spray called nasalCrom that helps me. If you have bad sinus issues you might want to check your house for mold too. I'm also looking into getting an ALF. Are you in good shoes?
The pattern is the pattern. There are compensations that can develop on top of the underlying pattern, but the pattern of right side dominance is still the foundation of it all since it's built into our body.

Another amazing video. Thank you
My left side is more dominant than my right side.
ive even taken measurements and each body part on my left side is about 2 cm bigger.
I even have four abs on my left side and only three on my right .
Thank you so much
Why does the masseter and temporalis tighten as well? Do you know the connection?... Thanks
hey, i get gas popping sound from my stomach when i flex my left side and breath at same time, can it put pressure on my organs when i squeez my left abs and rib too hard?
other wise thank you for your support, keep up the good work!
Elmar G Entertainment have the same question...
Neal...is the "gas popping" sound a result of achieving a domed left diaphragm, which in turn creates pressure on the fundus of the stomach which facilitates gastric emptying?
Please answer
me too, and i get a lot off pain in my stomach that lasts for days, please answer
same here man
What has temporarily helped with neck pain is sleeping with a smaller/lower pillow. I would wake up in so much neck pain and headaches. I stopped using two pillows and just used a smaller one and bingo.
That's good to hear. I have yet to find a comfortable pillow!
I still can;t find a good pillow.
Are you breathing upside down?
Rod, where do you live?
Albany Oregon. I was 180 pounds of pure muscle, in 2 years I’m at 140 and my right side has massive atrophy and I can’t lift my right arm. I think from breathing through my neck only all the muscles lifted my ribcage into my collar bones. I don’t know, but I’m in serious trouble my friend. I’ve seen countless doctors and chiros but nothing has helped.
Neal, I’m on Facebook and messenger as Rod Jaques, I can send you my cell number somehow as well. Even if you can’t treat me I would love someone to be able to talk to.
Neal Hallinan I must have replied to my own comment on accident, is there any way to get an assessment from you sir?
Neal Hallinan I live in Albany Oregon and I’m in serious trouble, please help me get on the right path of correction!!!
PLEASE Help! What xrays can i get to see this im so lost and in so much pain
It's $185 for him to look at your problem
Is it possible to be all tight on the right side but only have left sided body pain? From basically being spasmed in lengthening position (left side is where i only get pain but i KNOW my right side is tight and overactive although theres little Pain)
Yes this was my issue the sensation on the left for me was too much expansion
Bro can i talk with you
Is this why my right trap muscle always turns on when I talk or use my voice at all….?
my left hip is higher and right shoulder and dropped and I have more muscle on my left shoulder than my right any suggestions and my shoulders are uneven I first thought it was an imbalance and I would go to the gym and focus on the right shoulder but that wouldn't help in fixing the imbalance do you have any idea what is wrong structure and what is causing this and what I cant do to fix this?
You can check my website and try the beginner techniques. Just go slowly and read everything before you begin.

What if the tightness is on the left posterior neck and left upper thoracic area?
What would you look for if someone had Bilateral scapula cracking/popping when going from protraction to retraction and vice versa
I'd make sure your pelvis, ribcage, and neck are neutral. Then just follow standard PRI protocol/techniques. Neutrality has to come first.
@@NealHallinan is there any way to do at home testing on yourself, especially during these times?
So basically just work on left ZOA and that will inhibit the right side?
You could say that, yes. The acquisition of a left ZOA implies that the dominant right side has been inhibited. But if the right side doesn't inhibit, you can't get a left ZOA.
I have severe right abdominal pain ALL THE TIME. Spasms constantly in chest wall on both sides. Help!!!
Stretching right side lat would be helpful?
A little bit, but it's not enough by itself. You have to learn to actively shift into your left hip at the same time.
Which breathing technique help inhibit the right side ? My scalene and scms are constantly overactive and uncomfortable.
My scalenes and scms too, and the chest, how did you inhibit them and get the shoulder back?
Kresimir, read the answer I gave Jacob. It may apply to you, too.
yep me too, all my left side is jacked up, but i still feel like i do stand mostly on my right foot, its my left neck, shoulder, pec, lat, and rotator cuffs that are in pain and left side teeth that clench.
Will you ever be able to Get back the normal position of the temporal bone if you fix this??
yes, the temporal bones move. Externally rotating an internally rotated right temporal bone is part of the PRI process.
Neil i have left scm always coming, what does that mean?
I have arterial tos and am a baseball player. Pri has enabled me to throw a baseball again after two shoulder surgeries. However, I want to continue developing strength and power through weight training. Bench, specifically, is correlated with higher throwing and hitting velocities. I am wondering if it's possible to Bench and still reap the benefits of pri?
Hey Neil, what's your best definition of what it means to "inhibit" a particular muscle? Thanks!
To decrease the brain’s “need” to over-rely on certain muscle groups for breathing and stability.
I have left aic pattern and I always feel tightness on my left lats area after physical activities
So massage the subclavius is good enough to inhibit the right side? Its hard to get out of my right. Shoulld i massage scalene muscles as well?
No, it is not. You still need the techniques to help your brain "re-learn" how to not use the right subclavius for accessory breathing and stabilization.
Any tips to get air into right chestwall better than right lower back/abs. Even with 9090 with right arm overhead and ribs down air is still going into my right abs/lower back better than right chest
Have you tried the last technique on the page with a balloon?

This is great!
Thank you, Courtical Hoops.
I think my right pec is causing my diaphragm to be compressed and making my neck pop when I breathe deep
This is my exact situation except my scalene got over stretched from an injury, would this plan be the same for my issue ? Or because there’s an injury it’s different ?
I suffered a repetitive injury strain in my right QL and now I'm feeling pain and tightness in my right ribs/intercostals, glutes, mid back, SI. Would these types of exercises help? My right side is always tense and I feel like I've tried everything but it dosnt want to let go.
Also I'm a side sleeper, is there a certain side I should sleep on to help ease up the tightness?
Thanks!

Hi Neal. Is our entire right arm (down to the fingertips) affected too?
I had sent you an email, didn’t get response. Didn’t hear back. Hv polio with leg length discrepancy and my right side is weakened because it’s affected my hip and my right leg. I really need exercises to correct these issues. Can you help?
Any trainers you recommend in NYC?
Im very confused everything ur saying with the RIGHT is on my Left but I know I have the left aic thing am I wrong?
What do you recommend to do exactly to fix this?

Thank you
I hope you find a way to manage this,
Great Video, though there is something I don't understand.
AT pain can occur on either side. Lots of left AIC right BC patterns result in all left sided pain. Completely normal
Im getting left neck pain, could this be due to left aic pattern?
It can be because of the all three patterns, which all describe right sided dominance.
My left rib cage is higher than the right
That's normal in the right BC pattern.
Man when he was clanking the ribcage and pelvis models it felt like it hurt me
Hi Neal, as always great content!
Andrey Gladikov I experience the same thing
The Moldie Lyme how do you deal with it? Do you notice any food or caffeine relationship? In my case I believe it is a structural and chronic issue..
@@andreygladikov8860 A couple of options that have helped me if you have not tried are N-acetyl-cysteine and mouth taping at night. Also using horseradish or wasabi daily seems to keep things open
Andrew Dubé What kind of tape do you use?
Andrey Gladikov I have histamine intolerance issues. Usually anti-histamines help me. There's one nasal spray called nasalCrom that helps me. If you have bad sinus issues you might want to check your house for mold too. I'm also looking into getting an ALF. Are you in good shoes?
You are my hero
Is it the right UPPER trapezius or right LOWER trapezius that needs inhibition ?
Upper.
Hi. How would you know if one side is shifted differently like you are saying?
The pattern is the pattern. There are compensations that can develop on top of the underlying pattern, but the pattern of right side dominance is still the foundation of it all since it's built into our body.
I would like to give a huge "Thanks" to the 'Jaw physiotherapist' that stared blankly at me when I wondered if my tight neck that felt was "pulling" on my right jaw somehow could be involved in my hurting right jaw/TMJ. 'Not my area'. Hopefully the next generation of 'jaw'/'hip'/etc doctors will be schooled enough that they don't go 'Oh you have pain in your hip, that means you should see me and it means the hip is your problem - nothing else'. *mutters* =)
