the first verse is in the perspective of matt. he's saying that mr. rattlebone makes him feel better while also harming him physically. he brings him peace while also destroying him. the chorus is saying that mr. rattlebone(a personification of his addiction since being on drugs makes you emaciated) haunts him and has seen his darkest moments. the second verse is in the perspective of mr. rattlebone and is saying that hes now going to take over matts life and be a burden on him but at the same time put his pain at ease. the outro says "i am the driver" as in "i am in control." "i am the shadow" as in "i am the darkness that constantly follows you." "i am the hearse" as in "i control when/where/how you die." it can be argued that as the outro goes on it shifts from mr. rattlebone speaking to matt speaking, but thats fully debatable.

@@linalea04 only comment I have, apart from this being amazing, is that I do think the ending shifts from Mr. Rattlebones to Matt, as you can faintly hear it change from "I am the hearse" to "I'm in the hearse". Then the very last line is both Rattle and Matt, in my mind, singing together
