POV: How the Benzo Crisis Was Solved

  • In my journey through psychiatry training, I've noticed a troubling pattern: a reluctance to prescribe controlled medications and a stigma around benzodiazepines. This attitude has left patients feeling marginalized and struggling to access their essential medications. As a telepsychiatrist, I've seen firsthand the challenges patients face, from abrupt medication cuts to protracted withdrawal. Despite these perceived efforts to prevent a “benzodiazepine crisis” akin to another opioid crisis, policies often hinder access to proper care and almost seem to create a self-fulfilling prophecy. It's time for a shift in approach to prioritize patient well-being and improve education and accountability in prescribing practices. #mentalhealth #psychiatry #benzodiazepines #telepsychiatry

I recently moved to a new city, I’ve been finding it nearly impossible to find someone who will prescribe benzos. I’ve been on them for 22 years, I can’t just stop of course. I don’t want to be on them, especially at 60, but nothing else has helped my since I was 9. But I don’t have access to CPTSD therapy with Medicare/Medicaid. I have my records, I’ve never had any issues with lost prescriptions or anything like that, and I’m down to half of what I was taking 7 yrs ago. Every MH place I’ve called starts the convo with “the docs at this clinic don’t prescribe benzos” - before I say a word about what meds I’m on. Being on SSDI seems to mean I’m a drug-seeker here. I’m terrified.

😢 I laughed but then I got sad because when I was in the psychward, there was a girl there who went as voluntary, and she took 3 pills of Xanax per day during work hours... her psychiatrist took it off without tapering it and replaced it for another benzo. The girl started "going insane" and begged her to put on her Xanax again, the psychiatrist refused to do so! Fortunatly, she had pills at home and her boyfriend brought her without anyone knowing. I believe even voluntary entris are obligated to be there for at least 1/2 weeks. I'm not sure. I hope she is okay and had done a proper taper afterwards. I believe it was the same psychiatrist who took off a pill so the heart could pump blood, from an 60y/o lady and she passed out one day and hit with her cheekbone on the floor, had to do an x-ray. Fortunatly she didn't break any bone and that snake put her pill back without changing anything on her med intake. So it means it wasn't required to take off the heart pill! F*cking psychos! 🤬🤬🤬

I noticed a shift when my psychiatrist referred me to a therapist that suddenly became available in her office for the _meds she was prescribing_ me. I attended one ‘therapy’ session. It was one of the most demoralizing moments to be asked if I could restrain myself from taking benzodiazepines if they were on my bedside table, and I refused to answer. I refuse to be treated like an addict for a _dependency_ so I simply ignored the entire line of questioning and _found a new psychiatrist._

Another thing they are doing is, the insurance companies are refusing to pay for benzos and z drugs. You have to get a prior authorization and hope that the insurance company will approve it. Where I live, if insurance won’t pay, you’re not allowed to pay out of pocket either because it is a controlled substance.

This should make us not just think about the way we treat compliant benzo patients, but also the way we treat addicts. If you started the Xanax with an irresponsible physician or a guy selling bars on the corner, you started because you were trying to escape the pain of anxiety. I’m not suggesting that treatment should be the same, but I am suggesting that the end result is the same and that dependence should always yield compassionate care, not rapid withdrawal.

Yes! Brilliant…since they needed a pain medication temporarily but we also prescribed them an unnecessary combination of benzos, antidepressants, and muscle relaxers at the same time,…because “pain causes depression and depression causes pain”…if they do start having protracted withdrawal symptoms we can say it’s “drug seeking behavior” caused by the opiates and NOT the unnecessary drugs we prescribed 🤨
I can’t help but wonder how many people suffered from symptoms for years believing they were “drug addicts” because of their horrible symptoms … wicked.

To anyone tapering: you can do it, just take it slow.
I was kinda forced to start tapering after my primary doctor died from covid and the new doc said he wouldn't see me unless i started tapering off the 15mg/day valium I was on for over 10 years. Thankfully the new doc knew the dangers and worked with me if i needed more time after a step-down.
It wasn't easy, I lost my job and any social life I had. It took almost 2 years to complete the taper but it's nice to be free. My brain is still healing 18ish months after my last dose.
My next goal is to get off my SSRI that I've been on for the same amount of time but the nausea, vertigo and brain zaps are so brutal I'm scared to start the taper.
Sorry for the ramble, but if I can do it, anyone can, as long as you take your time.

This is so effed up that this is true. I’m so sick of our horrible medical system killing people. I had a Dr. Prescribe me this crap 20 years ago…. then when the opiate and benzo crisis hit, the doctor called me into his office and called me a drug addict, and I was so confused because I had taken the medication as prescribe, and then he started messing with the doses and the amount in each bottle every month and told me just to deal with it… and see look how anxious I was.
I told him - listen if you want me off the medication- we need to taper me off.
He made rude and inappropriate comments.- told me dress like his wife….
I found another Doctor Who was willing to prescribe it. I eventually got down to a quarter of the dose and now I’m going through withdrawals from going up and down with the does too much - in tiny increments - for the last two months in my life has become benzo hell. I’ve learned a lot in the past year about this medication and wish I had never started.
