    Sir 'fail exam' is common English it should be 'flunk exam'.

    1) Drive carefully lest you should / might have an accident.
    Or, Drive carefully lest you have an accident.
    Or, Drive carefully so that you might not have an accident.
    2) Practice well lest you should / might be defeated .
    Or, Practice well lest you be defeated.
    Or, Practice well so that you might not be defeated.

    Main sentence: "He told everything to the police lest he might be arrested."
    Dear Sir,
    I hope you are well. I am confused about the past tense. Would it be right or wrong if I make it in this way as per the "might have" rule above the sentence that has been written by you?
    "He told everything about the matter to the police lest he might have been arrested."

      This is a present subjunctive mood . So the ' might have been ' will be incorrect .
