Man when I played this game for the first time (I was 8) I DID NOT understand the implications of the maid's hotel. It's the creepiest part of the whole game IMO

That shadow in the Dead Hole... the silhouette is very similar to Oreko. I don't think it's little Sabitsuki or her sprite would resemble more the hospitalized children's. If it's really Oreko, could it mean that she was one of the Alleyway hospital's patients befriending Sabitsuki for a brief time... before ultimately dying of Sabitsuki's same illness with her body being cast into that hole? The object whom Sabitsuki's led to, deep down the hole, resembles Oreko's diving suit... The mystery deepens.

Makes me wonder if Oreko isn't wearing a diving helmet, but what a child Sabi interpreted as such. Perhaps they were childhood friends, considering Oreko seems more childlike than Sabi, and she disappears here...
Maybe she had a sort of breathing apparatus as a kid, and this is where Sabi last saw her?

I think it's Sister. My theory on this whole thing is that Sister was Sabi's mom, making Smile her uncle, but Sister died during childbirth, hence the part where child Sabi climbs out of the dead body of the black haired girl, why the Dying Girls event occurs, and why you beat up a bunch of black haired girls to get to it. The reason Sister died is because to be infected with Sabi's disease, you need to be genetically vulnerable to it, or be in contact with bodily fluids from the infected. The genes for such are recessive, so Sister and her husband were both "carriers", but Sabi was vulnerable, and infected before she was born, and she infected Sister, so that's what got her killed. Kaibutsu can only turn murderous once attacked after puberty, so Sabi couldn't turn until then, and Sister was monitored in the hospital after being infected, but that's why childbirth killed her. The event where Sabi sits at the table with Sister is her wishing that her mom was still alive. The part with Smile protecting Sister, and Smile killing her at the end of the Corrupted School is is Sabi wishing he had stopped her from killing Sister that comes from a combination of survivors guilt, childhood desires, and the knowledge that Smile hunts Kaibutsu. She also wants to die before she can hurt anyone else, like what happens in the Rusted School. If there's any holes in my theory, could someone point them out for me so that I can fix it? If so, thanks.

I always assumed it was the maid. First considering that you need to visit the maid in order for the scene to work. There is another "version" of the game where you play as child Sabi while she was at the hospital, and she is seemingly saved from death by the maid. Well, at least until... yea... but my theory was that the maid acted as something of a surrogate mother, at least until Rust came about.
