Mahito's form in this episode was gained through an immense understanding of his own soul. In exchange for not being able to use Idle Transfiguration on most of his body(except his hands), he gained a massive boost in his physical and defensive abilities thanks to a binding vow. It's stated that this form is about 200% tougher than his normal state. Yuji's hits that did so much damage before were suddenly having no impact on Mahito. It's stated in a little volume extra that if Yuji hadn't nailed Mahito with that last Black Flash, he would've been instantly torn to shreds. To me, one of the most interesting things about Mahito is how fast he grows. His potential. He's still essentially a child due to how young he is. Yet he managed to master Domain Expansion(something many sorcerers can never do), learned to transfigure himself more, found a counter to Simple Domain, used Black Flash, copied a very specific technique that GOJO of all people used on instinct by releasing his Domain for 0.2 seconds in order to counter Sukuna inside Yuji, told the King Of Curses to move aside, and discovered his true form thanks to realizing who he is. If he didn't have damage already accumulated in his soul or didn't have to worry about Sukuna being inside of Yuji, this wouldn't have even been a fight. Because he would've just used his more powerful soul techniques and just transfigured Yuji and then Todo. Mahito was the exact Curse that Sukuna said Jogo should've been. Someone who destroyed everything around him and lived only for himself and the torment of others. But in the end, Mahito's steps towards greatness were halted by a max power Black Flash straight to the soul. Centered on an unprotected part of his body. He managed to permanently cripple Todo and prevent him from using his technique ever again due to damaging his soul. And now, Geto has appeared before Mahito. Questioning if he will save him.
Shoko(the healer lady) can definitely heal back a limb but Mahito's ability damages more than than just the body, it destroys the shape of someone's soul and that's something that cannot be healed by a simple reverse cursed technique(the healing thing).
Mahito said during episode 12 of season 1 (when yuji tried to make sukuna heal junpei) that rct isn't something that can deal with the shape of the soul. Shoko heals through rct so she shouldn't be able to do anything for nobara or todo (or nanami but i don't think anybody is expecting something there...)
Gege uses the mahitos speech and yuji's "I am you" quote to draw parallels between mahito and yuji. Killing humans is as natural to Mahito as killing Curses is to Yuji. Hope that clears up any confusion a lot of people brushed that over
You guys should really rewatch the series dubbed. A lot of the jokes and phrases don’t translate into subtitles well, plus the English cast for this show did a really good job.

The domain of 0.2 seconds is just to be able to be safe from sukuna and hit todo faster

The body follows the shape of the soul. If the soul is gone, even Reversed Cursed Technique (which heals the body) can't heal you
