Sir can you please explain the concept of promontory? In this video, superior apex of the rhomboid which is point A was mentioned as the promontory which shows the point of maximum tension. In the simulation training video for Dufourmental flap, the promontory was mentioned as the point B or the point common to the flap and the defect. I got a little confused with this. Can we use this flap for the defects of the limbs, sir? For example, we have a motorcycle accident patient, he has the skin defect at his calf . It is round-shaped. Can we debride it into rhomboid defect and utilize Limberg flap ?
I have a patient. He has infectious acne on his leg, about 5cm proximal to his lateral malleolus. It has been 4 days since I debrided all festering tissue around infectious acne and it left a defect approximately 3x3 cm. Now I want to do a Limberg flap to cover the defect. But I hesitate to do that. I don't feel much safe because it is an infectious wound and the skin in this area is not much pliable. I would like to consult you and receive your precious advice about this issue. Can I contact with you via face book, sir? So that I can send you my patient's image for you to assess and discuss many other prospective cases in microsurgery and plastic in my clinical practice. I am very grateful to contact with you.
